43例脊柱结核的CT、MRI诊断分析

目的:探讨CT、MRI对脊柱结核的诊断价值.方法:收集43例经手术和病理证实的脊柱结核病例,对其CT和MRI表现进行回顾性分析.结果:43例中腰椎结核28例,胸椎结核16例,颈椎结核4例,骶尾椎结核3例.表现为椎体破坏,单个或多个椎体受累,多伴椎旁冷脓肿,可致椎管狭窄;部分椎间盘受累,表现为椎间隙变窄或消失;少数可累及附件.结论:CT的高密度分辨率和MRI的多轴位成像对脊柱结核有重要的诊断价值.CT、MRI能清楚显示椎体破坏及椎旁冷脓肿,了解病变范围及椎管有无狭窄,为临床选择治疗方案提供科学依据.

CT and MRI Diagnosis of Spinal Tuberculosis

Objective: To investigate the role of CT and MRI in diagnosis of the spine tuberculosis.Methods:To retrospectively study 43 postoperated patients,who all had the CT or MRI exams.All cases had histopathological evalutions.CT and MRI features were analysed retrospectively.Results:Of 43 patients,28 were tuberculosis in the lumbar vertebra,16 in the thoracic vertebra,4 in the cervical vertebra and 3 in the sacral and coccygeal vertebra.CT and MRI appearances were destruction of the vertebral body,single or multi centrum were involved.Abscess formation was seen in lots of cases,they may make the vertebral canal stricture.The intervertebral disc was involved to be narrow in part cases.Accessory parts can be destroyed in some cases.Conclusion:There is important diagnosis value for the spine tuberculosis making use of high density distinguish rate of CT and multi-axis imaging of MRI.They can show the destruction of vertebral body,the abscess around the vertebra,the metastasis area and stricture of the vertebral canal clearly.It provides scientific basis to choice treatment plan for clinic.
