原发性腹膜后肿瘤的诊治(附56例报告)

目的 探讨腹膜后肿瘤的诊断和治疗，提高其早期诊断及手术切除率。方法 分析我院56例原发性腹膜后肿瘤的临床表现、影像学特点及外科处理。结果 56例中良性肿瘤21例，完全切除15例（71．4％），合并脏器切除2例，大部切除4例，恶性肿瘤为35例，完全切除14例（40％），合并脏器切除12例，大部切除6例，手术探查活检3例，完全切除42例中术后复发22例（52．4％），再手术20例中13例肿瘤完全切除。结论 原发性腹膜后肿瘤早期临床表现缺乏特异性，诊断目前主要靠影像学检查，B超可作为首选检查手段，手术完整切除仍是治疗的主要手段，对复发性肿瘤应争取再次手术。

Diagnosis and Treatment of Primary Retroperitoneal Tumor: 56 Cases Report

Objective To explore of the diagnosin and management of primary retropeitoneal tumor and improve early diagnosis and radical resection. Methods Fifty-six cases of PRT were retrespectively analyzed in clinical manifestations, imaging findings and surgical managements. Results Tumors were benign in 21 patients and malignant in 35. Of the 21 patients with benign tumor, 15 underwent complete resection with incidental organ resection in 2 cases, palliative resection in 4 cases. Of the 35 malignant tumors, complete resection was performed in 14 cases, complete resection with incidental organs resection in 12 cases, biopsy only in 3 cases. After the total resection, 22 relapsed, then 20 cases received reoperation and 13 cases got total resection again. Condusions Because of lacking of early clinical manifestations, primary retreperitoneal tumors is now mainly diagnosised by imaging findings. B-ultrasound is a chief method in diagnosising, and complete resection is still a essential method of treatment. Recrudeacent tumor should strive for surgical operation.
