13例子宫角妊娠临床分析

目的 探讨宫角部妊娠的临床特征、诊断及治疗方法。方法 对无锡市第三人民医院2008年4月至2014年5月收治的13例子宫角妊娠患者进行回顾性分析。结果 13例子宫角妊娠患者中有明确停经史11例，不规则阴道流血6例，下腹痛6例，失血性休克4例。所有患者均予B超检查，其中发现宫角妊娠7例，5例误诊为异位妊娠，1例误诊为稽留流产，误诊率46．15％。3例在B超或腹腔镜下行清宫术，1例合并子宫腺肌症行全子宫切除术，9例行经腹或腹腔镜下一侧宫角楔形切除术。结论 宫角妊娠临床表现缺乏特异性，治疗以手术为主，部分病例也可选择保守治疗。

Clinical analysis of 13 cases of cornual pregnancy

Objective To study the clinical characteristics , diagnosis and treatment of cornual pregnancy .Methods A retrospective study was conducted on the clinical data of 13 cases with cornual pregnancy admitted and treated in Third People ’ s Hospital of Wuxi City from April 2008 to May 2014.Results Among the cases 11 patients had a clear history of amenorrhea , 6 patients presented with abdominal pain, 6 patients presented with vaginal bleeding and 4 patients became haemodynamically unstable .All patients were treated with B ultrasound examination , including 7 cases of cornual pregnancy .Five cases were misdiagnosed as ectopic pregnancy and 1 case was misdiagnosed as missed abortion , with misdiagnosed rate of 46.15%.Three cases received dilatation and curettage under B ultrasound or laparoscopy , one case combined with adenomyosis received hysterectomy and the other nine patients received cornual wedge resection by laparotomy operation or laparoscopic operation .Conclusion Cornual pregnancy lacks specific clinical features .Operation is the main treatment and conservative treatment can be used for some cases .
