功能锻炼与电刺激联合生物反馈对产后盆底功能障碍的疗效

摘    要:目的 探讨盆底肌功能锻炼与电刺激联合生物反馈对产后盆底功能恢复的临床效果。方法 选取2010年8月至2012年8月在湖北省应城市人民医院产科分娩的206例产后患有盆底功能障碍的产妇,随机分为对照组和观察组,对照组产妇给予常规盆底肌功能锻炼,观察组产妇在对照组的基础上给予电刺激联合生物反馈治疗,比较两组产后6个月产妇盆底肌收缩力和尿失禁情况,产后并发症及术后性生活。结果 观察组产妇盆底肌收缩力显著高于对照组(t =7.31,P<0.05),而尿失禁程度评分显著低于对照组(t=-2.73,P<0.05),观察组尿垫试验阳性率显著低于对照组(χ^2=9.75,P<0.05)。观察组产妇腰骶痛、子宫缩复不良、子宫后倾后屈、子宫脱垂、痔疮和阴道壁膨出的发生率显著低于对照组(χ^2别为3.54、15.73、4.29、11.62、7.76、6.14,均P<0.05)。对照组和观察组治疗后性生活水平均显著提高(χ^2别为17.30、121.69,均P<0.05),而治疗后观察组性生活水平较对照组显著提高(χ^2=2.48,P<0.05)。结论 盆底肌功能锻炼与电刺激联合生物反馈治疗有利于产后盆底功能的恢复,显著降低了产后盆底功能障碍的发生率。

Abstract:Objective To explore the clinical effect of pelvic floor muscle exercise combined with bio-feedback electric stimulation on recovery of pelvic floor function after delivery .Methods From August 2010 to August 2012, 206 primiparous women with pelvic floor dysfunction delivering in obstetrics department of the People ’ s Hospital of Yingcheng City in Hubei Province were divided into control group and observation group .The control group received pelvic floor muscle exercise , while the observation group received bio-feedback electric stimulation based on the treatment for the control group .Pelvic organs , postnatal complications and sexual life were evaluated 6 months after delivery .Results The pelvic floor muscle contraction force of the observation group was significantly higher than the control group (t=7.31, P〈0.05), but the incontinence score and the positive rate of urine pad test of the observation group were significantly lower than the control group (t=-2.73,χ^2=9.75, both P〈0.05).The incidence of lumbosacral pain, poor contraction of uterus, uterine retroversion retroflexion , uterine prolapse , hemorrhoids and vaginal wall prolapse was significantly lower in the observation group than the control group (χ^2value was 3.54, 15.73, 4.29, 11.62, 7.76 and 6.14, respectively, all P〈0.05).The sexual life of two group was improved (χ^2value was 17.30 and 121.69, respectively, both P〈0.05), and the improvement of the observation group was significantly greater (χ^2=72.48, P〈0.05).Conclusion Pelvic floor muscle exercise combined with bio-feedback electric stimulation is beneficial to recovery of pelvic floor function after delivery , which significantly reduces the incidence of pelvic floor dysfunction .
