成就动机策略在风湿免疫科护士同情心疲乏和道德困境中的中介效应

目的探讨成就动机策略在风湿免疫科护士同情心疲乏和道德困境中的中介效应。方法于2018年5—10月,采用一般资料调查问卷、成就动机量表、汉化版专业生活品质量表和中文版护士道德困境量表对新疆维吾尔自治区乌鲁木齐市6所三级甲等医院的275名风湿免疫科护士进行问卷调查。结果 257名风湿免疫科护士成就动机策略总分为5.45±1.58;专业生活品质总分为71.54±10.53;道德困境总分为166.15±53.17。护士成就动机与专业生活品质、道德困境各领域之间均呈显著负相关(P<0.05),专业生活品质、道德困境总分及各维度之间均呈显著正相关(P<0.05)。通过应用中介检验程序进行分析,成就动机策略在风湿免疫科护士同情心疲乏和道德困境中的中介效应所占比例为50.92%。结论成就动机策略显著地通过同情心疲乏影响着风湿免疫科护士的道德困境程度,护理管理者可以从提升护士追求成功的成就动机策略水平方面应对其可能会出现的同情心疲乏和道德困境危机。

Mediating effect of achievement motivation strategy on compassion fatigue and moral distress of nurses in department of rheumatological immunology

Objective]To investigate the mediating effect of achievement motivation strategy on compassion fatigue and moral distress of nurses in department of rheumatological immunology.Methods] By the General data questionnaire,achievement motivation scale,Chinese version professional quality of life scale and Chinese version of nurses’ moral dilemma scale,a questionnaire survey was conducted among 275 nurses of rheumatological immunology department in six grade-A tertiary hospitals in Urumqi of Xinjiang Uygur Autonomous region from May to October 2018.Results]The total scores of achievement motivation strategies,professional quality of life and moral dilemmas among 275 nurses of rheumatological immunology department were 5.45 ±1.58,71.54 ±10.53 and 166.15 ±53.17,respectively. Nurses’ achievement motivation was significantly negatively correlated with the professional quality of life and moral dilemmas(P<0.05). There were significant positive correlation between total scores of professional quality of life and moral dilemmas,as well as the dimensions of moral dilemmas(P<0.05). By using the intermediary test procedure,the achievement motivation strategy contributed 50.92% of intermediary effect between compassion fatigue and moral distress of nurses in department of rheumatological immunology.Conclusion]Achievement motivation strategies significantly affect the moral dilemmas of nurses in department of rheumatological immunology through compassionate fatigue. Nursing managers can deal with the possible crisis of compassion fatigue and moral dilemmas by raising the level of nurses’ achievement motivation strategies.
