创伤性动脉瘤的治疗

目的：探讨创伤性动脉瘤的外科手术治疗方法。方法：对近6年收治的创伤性动脉瘤39例进行回顾性分析。结果：24例行急诊手术治疗，动脉瘤切除后，10例作单纯动脉结扎，3例作动脉修补，26例用自体大隐静脉或人工血管移植重建血运，手术治愈37例，高位截肢2例，治愈率94．9％，术后随访26例（2个月－6年），均保存肢体。结论：外科手术仍是目前首选的外周创伤性动脉瘤治疗方法，应视具体情况选择手术方式，并重视感染性假性动脉瘤的治疗。

Surgical management of peripheral traumatic pseudoaneurysms

Objective To observe the efficacy of surgical management of peripheral traumatic pseudoaneurysms.?Methods The retrospective study was performed on 39 patients underwent surgical intervention for peripheral trauamatic pseudoaneurysms during the passed 6 years.?Results Emergent operations were performed in 24 cases.In addition to excision of the pseudoaneurysm,10 cases had simple ligation of artery,3 cases had patch angioplasty and the other 26 cases underwent revascularization procedure with autogenous greater saphenous vein or artificial vascular graft.Thirty four peripheral arterial pseudoaneurysms were cured and two major amputations were performed. The total successful rate was 94.9% .No delayed limb loss was found during follow up period(ranged 2 months 6 years) among 26 cases.?Conclusion At present surgical operation is the first option in treatment of peripheral traumatic pseudoaneurysm,and the surgical procedure should be chosen according to condition of the individual case.The importance of the treatment of infected peripheral arterial pseudoaneurysm should be emphasized.
