恶性胸膜间皮瘤40例临床分析

目的：探讨恶性胸膜间皮瘤的病因、临床特点、诊断、治疗及预后。方法：对1970年1月至2000年6月间收治的40例恶性胸膜间皮瘤的病人进行回顾性分析。结果：2例有石棉接触史，6例病人出现远处转移，转移的部位分别为肺、腹膜后、腋下。13例病人行B超引导下胸膜穿刺活检。4例病人接受放射治疗。结论：从本组病例不能看出恶性胸膜间皮瘤的发病同石棉接触之间的联系。B超引导下胸膜穿刺活检的阳性率可达。70％，可作为本病的首选诊断方法。出现转移的病人预后极差。放疗有助于延长提高病人的生存期，但因病例较少尚需做进一步临床研究。

Clinical analysis of 40 patients with malignant pleural mesothelioma

Objective: To investigate the causes,clinical characters,diagnosis, treatment and prognosis of malignant pleural mesothelioma.Methods: A retrospective study was conducted in 40 cases of malignant pleural mesothelioma hospitalized from Jannuary 1970 to June 2000.Results:Two of the 40 cases had definite history of exposure to asbestos.Metastasis appeared in lung,retroperitonum and axillary in 6 cases. B-ultrosound guided needle biopsy was conducted in 13 cases.4 patients received radiotherapy.Conclusion:Exposure to asbestos or not has no difinite relations to malignant pleural mesothelioma.Patients with distant metastasis had a bad prognosis.As the first diagnostic method,B-ultrosound guided needle biopsy of plura achieved a diagnostic positive rate of 70%.It seemed that radiotherapy was helpful to improve the survival time,but it need further clinical studying.
