超敏C反应蛋白与2型糖尿病并发血管病变

目的了解单纯2型糖尿病患者和并发血管病变者血清超敏C反应蛋白(hsCRP)水平的差异,探讨hsCRP对2型糖尿病并发血管病变的预测价值。方法于2012年2月至6月连续收集320例2型糖尿病患者病历资料,分为单纯糖尿病组和糖尿病并发血管病变组,收集对象身高、体质量、腰围、臀围、血压、糖尿病病程等资料,计算体质量指数(BMI)、腰臀比(WHR),检测hsCRP、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)和糖化血红蛋白(HbA1c)等指标。结果与单纯糖尿病组相比,糖尿病并发血管病变组糖尿病病程较长(t=2.69,P<0.01)、收缩压和舒张压水平均较高(t=2.59,2.60,P均<0.01)、hsCRP水平较高(t=2.78,P<0.01);Logistic回归分析显示,在平衡了其他因素的作用后,hsCRP仍为糖尿病并发血管病变的危险因素(OR=1.65,P<0.01)。结论 2型糖尿病并发血管病变时血清hsCRP表达水平高,高水平hsCRP是2型糖尿病并发血管病变的独立危险因素,可作为2型糖尿病并发血管病变的预测因素。

High-sensitivity C-reactive protein and type 2 diabetes mellitus with vasculopathy

Objective To investigate the difference of high-sensitivity C-reactive protein（hs-CRP） level between pure type 2 diabetes mellitus and type 2 diabetes mellitus with vasculopathy,and to explore the predicting value of hs-CRP on type 2 diabetes mellitus with vasculopathy.Methods Between February and June 2012,320 patients with type 2 diabetes mellitus were successively collected and divided into pure diabetes and diabetes with vasculopathy groups for their data of height,weight,waistline,hipline,blood pressure,disease course.Thereafter,the body mass index（BMI） and waist hip rate（WHR） were examined.The indications including hsCRP,fasting blood-glucose（FBG）,total cholesterol（TC）,triglyceride（TG）,high-density lipoprotein cholesterol（HDL-C）,low density lipoprotein cholesterol（LDL-C） and glycosylated hemoglobin（HbA1c） were detected.Results As compared with pure diabetes group,diabetes with vasculopathy group had relatively longer disease course（t=2.69,P〈0.01）,higher levels of systolic and diastolic pressures（t=2.59 plus 2.60,P〈0.01）,higher hsCRP level（t=2.78,P〈0.01）.Moreover,Logistic regression analysis showed that hsCRP remained to be the risk factor of diabetes with vasculopathy（OR=1.65,P〈0.01）.Conclusion The serum hsCRP expression level of diabetes with vasculopathy was increased when lesions occurred.The hsCRP with high expression level was the independent risk factor to cause lesion of diabetes with vasculopathy hence the predicting factor for type 2 diabetes mellitus with vasculopathy.
