多层螺旋CT三维重建在周围型肺小腺癌诊断中的初步临床应用

目的 初步探讨多层螺旋CT三维重建对周围型肺小腺癌的诊断价值。方法 24例患者行多层螺旋CT薄层容积扫描，经工作站后处理行三维重建，容积显示重建（VR），将肿瘤影像与临床病理对照研究。结果 24例肺周围型小腺癌的密度改变表现为两种类型；非实质性密度影（磨玻璃影和空泡征），实质性密度影，4例细支气管肺泡癌和1例高分化腺癌均表现为非实质性密度，而6例中-低分化和低分化腺癌中5例呈实质性密度影，24例VR图像可以清晰显示病灶的整体轮廓以及其周围的血管集聚情况，其中5例利用CT容积测定软件准确测量肿瘤的体积大小，为随诊观察提供可靠依据。结论 多层螺旋CT薄层扫描及三维重建对周围型肺小腺癌极具诊断价值。

Preliminary Clinical Experience of 3-Dimensional Reconstruction of Multi-Slice Spiral CT in the Lung Peripheral Small Adenocarcinoma

Objective To study the value of three dimensional reconstruction of multi slice spiral CT in diagnosing small adenocarcinoma of peripheral lung. Methods Twenty four patients underwent thin multi slice spiral CT scan.Images were postprocessed in working station,including volume reconstruction (VR).The image characteristics of the tumors assessed by multi sllice CT were correlated with the pathological manifestation. Results In 24 cases of small adenocarcinoma of peripheral lung,the tumor density had two forms:non solid attenuation (ground glass opacity and air foam sign),and solid attenuation.Four cases with bronchioloalveolar carcinoma (BAC) and 1 case with higher differentiated adenocarcinoma had non solid attenuation,while 5 cases in 6 cases with lower adenocarcinoma had solid attenuation.VR images in 24 cases could clearly show the whole outline of neoplasms and peripheral collected vessels.Five cases were measured the volume of the whole focus by CT measuring volume software,offering reliable foundation for reexamine. Conclusion Thin scan and three dimensional reconstruction with multi slice spiral CT can play an important role in the diagnosis of small lung adenocarcinoma.
