单人调剂处方的风险防范实例分析

目的:探讨如何降低单人调剂处方的风险,以提高药学服务质量。方法:根据我院单人调剂处方引起纠纷的实例,分析其产生调剂风险的原因并提出建议。结果与结论:药师服务观念、处方调剂错误、药学知识缺乏、不良工作习惯等均可引发调剂风险。药师转变服务观念、采用"双容器"取药法调剂药品、加强药学知识的学习、培养良好调剂与卫生习惯及配备适当的硬件、加强"医、护"审核等可有效防范或降低单人调剂处方风险的发生。

Precautions against the Risk of Individual Drug Dispensing

OBJECTIVE： To investigate how to reduce the risk of individual drug dispensing, and to improve the quality of pharmaceutical care. METHODS： According to practical cases of disputes resulted from the individual dispensing in our hospital, the reasons for the risks of drug dispensing were analyzed to put forwards some suggestions. RESULTS ＆ CONCLUSIONS： Pharmacist service concept, dispensing error, pharmaceutical knowledge insufficiency, bad work habit, and so on, can induce the risk of drug dispensing. The article provides six suggestions to prevent or reduce the risk of individual drug dispensing in hospital pharmacy, The six suggestions include changing service conception of pharmacist, conducting drug dispensing by double container, enforcing learning, developing good dispensing and healthy habits, setting up suitable medicine board equipment, and strengthening physician and nurses examination.
