围术期预防用抗菌药物干预管理模式的建立与推广

目的:为规范围术期预防用抗菌药物探索合理的干预管理措施。方法:建立围术期预防用抗菌药物干预管理模式并进行推广应用;选取我院2010年3、6、9月的11种清洁切口手术出院病例,进行干预前、后预防用抗菌药物合理性的对照研究。结果:通过成立抗菌药物干预协作组织、制定管理制度和实施细则、完善抗菌药物知识培训与临床药师干预机制等措施和实施连续性循环干预,我院干预前、后预防用抗菌药物合理率从干预前的12.38%分别提升至干预后的33.99%和51.21%(P<0.05)。结论:我院建立的围术期预防用抗菌药物实施细则及采取的综合干预管理模式切实可行,可有效提高围术期抗菌药物的合理应用率。

Establishment and Promotion of Intervention and Management Model for Perioperative Prophylactic Application of Antibiotics

OBJECTIVE： To standardize perioperative prophylactic application of antibiotics and explore reasonable intervention measures. METHODS： The intervention and management model of perioperative prophylactic application of antibiotics were established and promoted; and 620 discharged cases of 11 types of clean incision operations were selected from our hospital in Mar., Jun. and Sept. in 2010, and the rationality of prophylactic application of antibiotics was compared before and after intervention. RESULTS： Through establishing a management group of antibiotics, developing management systems and implementation rules, improving antibiotics knowledge training and mechanism of intervention made by clinical pharmacists and carrying out consecutive intervention cycle, rationality rate was improved from 12.38% before intervention to 33.99% and 51.21% after intervention （P〈 0.05）. CONCLUSIONS： It is feasible and effective to establish implementation rules and management measures for perioperative prophylactic application of antibiotics in our hospital. It is effective to improve rational use of antibiotics.
