早期留置鼻空肠管肠内营养在重症急性胰腺炎中的临床研究

目的评价早期放置鼻空肠营养管行肠内营养支持在重症急性胰腺炎(SAP)治疗中的作用。方法回顾分析该院近3年来收治的25例SAP患者,12例早期接受经鼻空肠管肠内营养+肠外营养(EN+PN)治疗,13例接受全肠外营养(TPN)治疗,比较两组患者腹胀缓解时间、血淀粉酶恢复正常的时间、住院天数、并发症的发生率及感染率的差异。结果经鼻空肠管肠内营养+肠外营养组(EN+PN)与全肠外营养(TPN)支持组腹胀缓解时间、血淀粉酶恢复正常的时间、住院天数、并发症的发生率及感染率相比差异有显著性(P<0.05)。结论早期经鼻空肠管EN+PN治疗重症急性胰腺炎能改善全身情况,安全可行,值得临床推广。

Clinical study of early enteral nutrition on treatment of severe acute pancreatitis through nose-jejunum tube

Aim To investigate the role of early enteral nutrition on treatment of severe acute pancreatitis through nose-jejunum tube.Methods The data of 25 patients with severe acute pancreatitis（SAP） in past three years were retrospectively analyzed.The patients were divided into two groups： EN＋PN group consisted of 12 patients receiving early enteral nutrition through nose-jejunum tube and parenteral nutrition therapy,and TPN group consisted of 13 patients receiving total parenteral nutrition therapy.The differentiation of the abdominal distension relief time,the serum amylase recovery time,the length of hospital stay,the incidence of complication and the infection rate were compared between the two groups.Results Compared with TPN group,there was a highly significant reduction in the abdominal distension relief time,the serum amylase recovery time,the length of hospital stay,the incidence of complication and the infection rate in EN＋PN group（P0.05）.Conclusion The early EN＋PN on treatment of severe acute pancreatitis through nose-jejunum tube is safe,effective and worthy to be popularized,which can ameliorate body state.
