雾化吸入联合振动排痰在尘肺大容量全肺灌洗治疗中的应用体会

摘    要:目的 观察氧气驱动雾化吸入普米克令舒、万托林联合振动排痰在尘肺大容量全肺灌洗治疗中的作用,探讨尘肺手术患者有效的护理、治疗措施.方法 尘肺患者60例,常规WLL组30例行常规大容量全肺灌洗,给予常规护理,治疗组30例在常规治疗的基础上辅以雾化吸入联合振动排痰.两组患者术前6小时内常规给予预防抗感染,术后根据情况决定是否继续预防性抗感染治疗.比较两组术后肺功能改善和临床症状综合评分变化及平均使用抗生数天数、治疗总费用.结果 与常规WLL组相比,治疗后两组肺功能及临床症状均较术前改善,以治疗组改善更为显著(P〈0.05),治疗组使用抗生素时间缩短,治疗总费用无增加.结论 氧气驱动雾化吸入普米克令舒、万托林联合振动排痰有利于尘肺大容量全肺灌洗患者术后恢复,有效减少了抗生素的使用而未增加患者的花费,值得临床推广.

Abstract:Objective To evaluate the significance of aerosol inhalation combined with expectoration by vi- bration in massive whole-lung lavage of pneumoconiosis. Methods 60 patients with pneumoconiosis were divided in- to two groups. The control group was given conventional treatment, and the treatment group was additionally treated with expectoration by vibration. All the patients were given anti-infection prevention 6 hours before surgery. The ther- apeutic effect of the two groups was evaluated. Results There were statistically significant differences in lung func- tion and improvement of clinical symptoms between the two groups ( P 〈 0.05 ). The average time of antibiotics use was shorter in the treatment group than in the control group, and the total cost of the two groups was the same. Con- clusion Oxygen driven aerosol inhalation combined with expectoration by vibration could accelerate their postopera- tive recovery and shorten the average time of antibiotics use with low cost in the treatment of massive whole-lung lay- age of pneumoconiosis.
