学龄前儿童肺炎支原体肺炎临床特点分析简

目的 探讨学龄前儿童肺炎支原体肺炎的临床特点.方法 学龄前儿童肺炎支原体肺炎187例,按年龄分为5岁以上及5岁以下两组,并对其临床症状、影像学检查、实验室检查结果以及治疗方法及预后进行分析.结果 学龄前儿童MPP的表现中以5岁以上患儿临床表现更明显,5岁以下患儿肺部体征往往较为明显.胸部X线病变类型以肺间质浸润为主,5岁以下患儿支气管肺炎较5岁以上多见,而5岁以上患儿大叶性肺炎的发病率较高.发病一周以上者送检血清MP-IgM的阳性率,5岁以上患儿显著高于5岁以下;所有病例均使用大环内酯类抗生素治疗,治疗有效.结论 学龄前组儿童随年龄变化临床表现差异较大,尽早完善检查,明确诊断,及早治疗,特别是对于感染后首次发生喘息或反复喘息的小于3岁的患儿,要想到支原体感染的可能.

Clinical features of preschool-aged children with mycoplasma pneumoniae pneumonia

Objective To investigate the clinical features of mycoplasma pneumoniae pneumonia （MPP） in preschool-aged children. Methods 187 preschool-aged children with MPP were divided into two groups based on their ages, the group A older than 5 years old and the group B younger than 5 years old. Their clinical manifestation, radiographic findings, laboratory examinations, curative effect and sequelae were analyzed. Results The clinical manifestation was more obvious in the grnup B than in the grnup A, and the pulmonary signs were more obvious in the group A than in the group B. The chest X-ray fimtings mainly included predominant interstitial lung infihration. Bronchopneumnnia mainly occurred in the group A, and lobar pneumonia mostly occurred in the group B. The posi- tive rate of serum MP-IgM was higher in the group A than in the group B. All patients were treated with macrolide an- tibiotics, with good curative effect. Conclusion The clinical features of preschool-aged children with MPP vary a- long with the age changes. They should be given complete detection, definitive diagnosis and early treatment as soon as possible.
