自荧光气管镜对中心型肺癌的诊断价值

摘    要:目的 研究自荧光支气管镜(AFB)对中心型肺癌的诊断价值.方法 2011年11月至2013年2月我院对36例中心型肺癌患者进行白光支气管(WLB)和AFB检查,其中男32例,女4例;年龄(58.56±9.84)岁.经鼻插入气管镜,先在白光下进行观察,再切换至荧光下进行观察,在白光下对所有标记的可疑部位(包括白光和荧光下记录)进行活检,并取白光和荧光下正常部位活检进行对照.结果 共取活检180处,去除原发部位36处外,剩余144处活检进行统计处理.WLB和AFB对肺癌诊断的敏感性分别为62.5%、86.49%(P=0.0295),特异性分别为58.33%、85.57%(P=0.0027).结论 AFB对中心型肺癌诊断的敏感性高于WLB,能够指导临床治疗.

Abstract:Objective To investigate the diagnostic value of auto-fluorescence bronchoscopy in central type lung cancer. Methods 36 patients with central type lung cancer were given WLB and AFB procedure, which in- cluded 32 male and 4 female patients. Their average age was 58.56 ± 9. 84 years old. Each patient underwent WLB was followed by AFB procedure, All of the suspicious abnormal sites were biopsied for pathological examination, and also from normal control sites. Results 180 specimens were obtained, and there were 144 specimens were statisti- cally analyzed. In the diagnosis of lung cancer, WLB had a sensitivity of 62. 5% and a specificity of 58. 33%, and AFB had a sensitivity of 86.49% and a specificity of 85.57% respectively. Conclusion AFB has a higher sensitivi- ty in the diagnosis of patients with central type lung cancer than than WLB dose.
