肿瘤外科实施“医药护”责任一体化的作用及效果分析

目的：探讨临床药师促进抗菌药物在肿瘤外科合理应用的效果.方法：我院自2012年5月在肿瘤外科实施“医药护”责任一体化试点工作,临床药师以抗感染药为切入点参与临床实践.利用医院信息系统（HIS）,调取我院2011年与2012年抗菌药物使用数据;同时调取肿瘤外科2012年1-7月抗菌药物使用数据,调查抗菌药物使用率、抗菌药物使用强度;调取肿瘤外科2012年1-7月Ⅰ类切口手术病历,调查Ⅰ类切口手术抗菌药物应用情况,并与2011年1-7月相关数据进行对比、分析.结果：抗菌药物使用率由2012年1月的41.72％降至7月的27.96％,Ⅰ类切口手术抗菌药物预防性使用率由2012年1月的100％降至7月的5.56％.结论：通过肿瘤外科临床药师的药学实践,可达到逐步规范专科抗菌药物合理应用的目的.

EffectivenessofClinician-Pharmacist-NurseResponsibilityIntegrationinSurgicalOncologyDepartment

OBJECTIVE：To investigate the effectiveness of clinical pharmacists in promoting rational use of antibiotics in surgical oncology. METHODS ： The pilot work of clinician-pharmacist-nurse responsibility integration was implemented in surgical oncology department of our hospital since May 2012 and clinical pharmacists participated in clinical practice with anti-infective drugs as starting port. The consumption data of antibiotics in our hospital from 2011 to 2012 were retrieved from HIS system; the consumption data of antibiotics during Jan. -Jul. period of 2012 in surgical oncology department were retrieved regarding the rate of antibiotic use, antibiotic use density （AUD） ; the surgical records of patients undergoing type I incision operation during Jan. -Jul. period of 2012 was reviewed for information of the antibiotic use for type I incision patients and the data were compared contrastively with those during Jan.-Jul. period of 2011. RESULTS： The rate of antibiotic use in July 2012 reduced from 41.72% in January to 27.96% in July. The rate of using prophylactic antibiotics in patients undergoing type I incision operation in 2012 reduced from 100% in January to 5.56% in July. CONCLUSION： Pharmacists＇ pharmacy practice in surgical oncology department contributed to gradual standardization of rational use of antibiotics in surgical oncology department.
