不同年龄段结节性甲状腺肿合并甲状腺癌患者超声及造影特征分析

目的探讨不同年龄段结节性甲状腺肿伴甲状腺癌患者的超声影像特征。方法选择2015—2019年在北票市中心医院超声科进行检查的258例结节性甲状腺肿伴甲状腺癌患者为观察组,依据患者年龄将观察组分为低年龄组126例、中年龄组73例和高年龄组59例,另选同期63例单纯性甲状腺癌患者为对照组,比较各组间超声声像特征。结果低年龄组、中年龄组和高年龄组病灶回声、钙化和血流分布差异有统计学意义(P<0.05)。对照组和观察组各亚组之间平均收缩期流速、阻力指数差异有统计学意义(P<0.05),造影剂平均渡过时间差异无统计学意义(P>0.05)。结论低年龄段结节性甲状腺肿伴甲状腺癌患者的超声声像特征与单纯性甲状腺癌较为接近,主要表现为病灶回声低、伴微小钙化多,病灶中心血供丰富、血流速度快、阻力大。

Analysis of Ultrasound and Contrast-enhanced Ultrasound Features in Patients with Nodular Goiter Complicated with Thyroid Cancer at Different Ages

Objective To explore the ultrasound and contrast-enhanced ultrasound characteristics of nodular goiter complicated with thyroid cancer in different age groups.Methods A total of 258 patients with nodular goiter complicated with thyroid cancer who were examined in ultrasound department from 2015 to 2019 in Beipiao Central Hospital were selected as the observation group.According to the age of patients,the observation group were divided into the low age group(126 cases),the middle age group(73 cases)and the high age group(59 cases).The 63 cases with thyroid cancer were selected as the control group at the same term.The ultrasonic image characteristics were compared among groups.Results There were significant differences in echo,calcification and blood flow distribution among low age group,middle age group and high age group(P<0.05).The difference of mean systolic velocity and resistance index between the control group and the observation group was significant(P<0.05),but the difference of mean time of passage of contrast medium was not significant(P>0.05).Conclusion The ultrasonic image characteristics of nodular goiter complicated with thyroid cancer in low-age group were similar to those of thyroid cancer patients.The main manifestations are low echo,multiple microcalcifications,rich blood supply in the center of the focus,fast blood flow and high resistance.
