丙型肝炎患者血清和外周血单个核细胞HCV-RNA检测

目的 :了解丙型肝炎患者血清和外周血单个核细胞 (PBMC)中HCV RNA存在情况及其意义。方法 :采用套式PCR检测 38例急性丙型肝炎和 36例慢性丙型肝炎患者血清和外周血PBMC中HCV RNA。结果 :74例丙型肝炎患者血清HCV RNA阳性 6 8例 ,PBMC阳性 44例 ;急性丙型肝炎患者PBMC中HCV RNA阳性 15例 (39.5 % ) ,慢性丙型肝炎患者PBMC中阳性 2 9例 (80 .6 % )。慢性丙型肝炎患者PBMC中HCV RNA检出率显著高于急性丙型肝炎患者 (P <0 .0 1)。 3例血清HCV RNA阴性 ,而PBMC中HCV RNA均阳性 ;8例患者血清抗 HCV阴性 ,而血清HCV RNA均阳性。结论 :PBMC可能为HCV肝外贮存和复制场所 ,PBMC中的HCV感染在丙型肝炎慢性化和慢性肝损害中可能起一定作用 ;血清HCV RNA检测有助于抗 HCV阴性丙型肝炎的诊断。

The detection of HCV-RNA in serum and peripheral blood mononuclear cells of patients with hepatitis C

Objective:To investigate the existence of hepatitis C virus(HCV) RNA in serum and peripheral blood mononuclear cells(PBMC) of patients with hepatitis C and to explore its clinical significance.Methods:Nested polymerase chain reaction(PCR) was applied to detect HCV RNA both in the sera and in PBMC of 38 patients with acute hepatitis C and 36 patients with chronic hepatitis C.Results:It was found that of total 74 cases,68 were HCV RNA postitive in serum,44 were HCV RNA positive in PBMC. Of patients with acute hepatitis C,15(39.5%) were HCV RNA positive in PBMC,meanwhile 29(80.6%) were HCV RNA positive in PBMC among chronic hepatitis C. The incidence of HCV infection of PBMC is significantly higher in chronic hepatitis C than that of acute hepatitis C( P <0.01).Three patients with negative RNA in their sera showed HCV RNA positive in PBMC. Eight patients with negative anti HCV was confirmed to be hepatitis C by detecting positive HCV RNA in their sera. Conclusions:It is possible that PBMC might be the extra hepatic resort for HCV replication and storage. The HCV infection of PBMC might play certain role in chronicity and long term liver damage of hepatitis C;The detection of serum HCV RNA might have important significance in the diagnosis of anti HCV negative hepatitis C.
