31例青少年腮腺肿瘤临床和病理分析

目的 探讨青少年腮腺肿瘤的临床和病理特点,为其治疗提供临床证据. 方法 对31例10~19岁青少年腮腺肿瘤患者的临床和病理资料进行回顾性分析,并将其与同期收治的516例成人腮腺肿瘤患者的临床特点和组织病理类型加以比较. 结果 青少年腮腺肿瘤中恶性肿瘤比例高达38. 7%(12/31),但在成人仅占17. 0%(88/516). 31例青少年腮腺肿瘤中,良性肿瘤19 例,占61. 3%,其中以多形性腺瘤居多,占良性肿瘤的89. 5%(17/19);恶性肿瘤中以黏液表皮样癌居多,占恶性肿瘤的41. 7%(5/12). 31例肿瘤患者均接受手术治疗,其中2例恶性肿瘤术后放疗. 结论 青少年腮腺肿瘤的肿瘤病理类型与成人有明显不同,应当引起重视. 对于青少年腮腺肿瘤,应重视手术治疗的早期性和彻底性.

Clinical and pathological analysis of parotid gland tumor in adolescent: report of 31 cases

Objective To investigate the clinical and pathological characteristics of the parotid gland tumor in adoles-cent for providing clinical evidence for its treatment. Methods The clinical and pathological data of 31 adolescents ( 10-19 years old) from Jan. 2003 to Dec. 2011 were retrospectively studied and compared with 516 adult cases at the same time. Results 38. 7%( 12/31) of the parotid gland tumors in adolescent were malignant, but only 17%( 88/516) in adults. Among the 31 cases, 19 cases were benign tumor( 61. 3%) , 12 cases were malignant tumor( 38. 7%) . Among the benign cases, pleomorphic adenoma was the most common diseases, which accounted for 89. 5%( 17/19) . Mucoepi-dermoid carcinoma( 5/12) was the most common malignancy. All of the 31 cases were treated surgically, and 2 malignant cases were treated combined with postoperative radiotherapy. Conclusion The clinical and pathological features of pa-rotid gland tumors in adolescent are different from those in adults, and more attention should be paid to this phenomenon. Operative treatment is the preferred treatment of parotid gland tumor in adolescent, which should be early and thorough. Thus far, there is great dispute on postoperative radiotherapy.
