HE4、OPN、MSLN水平对上皮性卵巢癌的诊断价值

目的 探讨血清人附睾分泌蛋白4(HE4)、骨桥蛋白(OPN)及间皮素(MSLN)水平对上皮性卵巢癌(EOC)的诊断价值.方法 选择100例EOC患者(EOC组)、60例卵巢上皮良性肿瘤患者(良性组)和60例健康女性体检者(健康组),检测3组研究对象的血清HE4、OPN及MSLN水平,分析3种指标单独及联合诊断EOC的临床价值.结果 EOC组患者的血清HE4、OPN、MSLN水平均高于良性组和健康组,差异均有统计学意义(P﹤0.05);良性组和健康组的血清HE4、OPN、MSLN水平比较,差异均无统计学意义(P﹥0.05);血清OPN+HE4+MSLN正确诊断EOC患者92例,诊断灵敏度为92.00%,特异度为90.83%,漏诊率为8.00%,误诊率为9.17%,曲线下面积(AUC)为0.916.结论 血清OPN+HE4+MSLN联合检测对EOC具有较高的诊断价值,较单一指标检测提高了灵敏度和特异度.

Clinical application of HE4,OPN and MSLN in the differential diagnosis of epithelial ovarian cancer

Objective To investigate the clinical value of serum human epididymal secretory protein 4(HE4),osteo-pontin(OPN)and level of mesothelin(MSLN)in the differential diagnosis of epithelial ovarian cancer(EOC).Method 100 cases with EOC (EOC group), 60 cases of benign epithelial ovarian lesions (benign lesions group) and 60 healthy women (healthy subjects group) were included in the analysis, the serum HE4, OPN and MSLN level in these patients were detected,and the clinical value of the three indicators alone or combined in the diagnosis of ovarian cancer were ana-lyzed.Result In EOC group,the serum HE4,OPN and MSLN levels were significantly higher than that in benign le-sions group and healthy subjects group,the difference was statistically significant(P<0.05);while there was no statistical-ly significant difference in benign lesions group and healthy subjects group in regard of serum HE4,OPN,and MSLN lev-el(P>0.05);serum OPN+HE4+MSLN correctly diagnosed 92 cases of EOC,and the sensitivity was 92.00%,specificity was 90.83%,with misdiagnosis rate being 8.00%,and rate of missed diagnosis being 9.17%,the AUC was 0.916.Conclu-sion Combined detection of serum OPN+HE4+MSLN is of high diagnostic value in detecting EOC as compared with each indicator utilized along,and it can significantly improve the sensitivity and specificity of diagnosis.
