穴位针灸联合膀胱康复训练对脊髓损伤伴神经源性膀胱病人预后的影响

目的:探讨穴位针灸联合膀胱康复训练对脊髓损伤伴神经源性膀胱病人预后的影响。方法:选取128例脊髓损伤伴神经源性膀胱病人，根据随机数字表将病人分为观察组和对照组，各64例，对照组病人应用间歇性清洁导尿及膀胱功能训练指导，观察组在对照组基础上应用穴位针灸，2组干预时间为8周，比较2组病人干预前后膀胱功能恢复及生活质量的变化。结果:2组病人干预后膀胱容量、自行排尿频次、残余尿量及导尿间隔时间均明显优于干预前（P<0.01），且观察组干预后膀胱容量、自行排尿频次、残余尿量及导尿间隔时间均明显优于对照组（P<0.01）。2组干预后躯体、社会、心理及物质功能等纬度评分较干预前明显提高（P<0.01），且观察组病人干预后躯体、社会、心理及物质功能评分均明显高于对照组（P<0.01）。结论:穴位针灸联合膀胱康复训练能有效改善脊髓损伤伴神经源性膀胱病人膀胱功能，可提高病人生活质量。

Effect of acupoint acupuncture combined with bladder rehabilitation training on the prognosis of patients with spinal cord injury complicated with neurogenic bladder

Objective:To investigate the effects of acupoint acupuncture combined with bladder rehabilitation training on the prognosis of patients with spinal cord injury complicated with neurogenic bladder.Methods:One hundred and twenty-eight patients with spinal cord injury complicated with neurogenic bladder were randomly divided into the observation group(n=64)and control group(n=64).The control group was treated with clear intermittent catheterization combined with bladder function training guidance,and the observation group was additionally treated with acupoint acupuncture based on the control group.The intervention time was 8 weeks,and the bladder function recovery and quality of life between two groups were compared.Results:After intervention,the volume of bladder,frequency of voiding,residual urine volume and interval of catheterization in two groups were significantly better than those of before intervention(P<0.01),and which in observation group were significantly better than those in control group(P<0.01).Compared with before intervention,the latitude scores of physical,social,psychological and material function in two groups were significantly improved after intervention(P<0.01),and which in observation group were significantly higher than those in control group after intervention(P<0.01).Conclusions:Acupoint acupuncture combined with bladder rehabilitation training can effectively improve the bladder function and quality of life in patients with spinal cord injury complicated with neurogenic bladder.
