下消化道出血血管造影诊断与介入治疗

目的:探讨下消化道出血介入诊断和治疗的临床价值。方法:对22例下消化道出血病人行数字减影血管造影（DSA）检查，对明确出血部位行超选择性动脉栓塞或经导管持续灌注血管加压素止血治疗。结果:22例病人中，诊断明确19例，DSA诊断阳性率为86.4%（19/22），其中，出血发生在空肠9例，回肠5例，结肠5例。18例成功止血，介入治疗成功率94.7%（18/19）。术后随访6~12个月，有2例再出血，病人均无严重并发症发生。结论:介入放射学对下消化道出血的诊断和治疗是安全有效的，特别是介入治疗具有独特的临床价值。

Angiographic diagnosis and interventional therapy of lower digestive tract hemorrhage

Objective:To explore the interventional diagnosis and treatment of lower digestive tract hemorrhage.Methods:Twenty-two patients with lower digestive tract hemorrhage were detected using the digital substrate angiography (DSA).The bleeding lesions were treated with superselective artery embolization or transcatheter infusion vasopressin.Results:Among 22 patients,19 cases were clearly diagnosed,the positive diagnosis rate of which was 86.4％ (19/22).Jejunum bleeding in 9 cases,ileum bleeding in 5 cases and colon bleeding in 5 cases were found.The hemostasis in 18 cases was successfully completed,the success rate of treatment was 94.7％ (18/19).All patients were followed up for 6 to 12 months,the rebleeding in 2 cases were identified,and no severe complication was found.Conclusions:The interventional radiology in the diagnosis and treatment of lower digestive tract hemorrhage is safe and effective,especially,the interventional therapy has unique clinical values.
