匹多莫德佐治儿童大叶性肺炎支原体肺炎的疗效评价

摘    要:目的:观察匹多莫德佐治儿童大叶性肺炎支原体肺炎的临床效果,评价其对细胞免疫功能的影响.方法:选取48例大叶性MPP患儿随机分为A、B组,同时选取门诊同期体检的健康儿童30名为对照组.B组予以常规阿奇霉素(10 mg·kg-1·d-1,3个疗程)治疗,A组予以阿奇霉素联合匹多莫德(0.4 g/d,2个月)治疗,采用流式细胞仪检测对照组及A、B组患儿治疗前、治疗2个月后外周血T淋巴细胞亚群的变化,并且比较A、B组的临床效果.结果:治疗前,大叶性MPP组与对照组患儿血清CD3+、CD4+、CD4+/CD8+比例明显降低,CD8+比例升高(P<0.01).治疗后,患儿血清CD3+、CD4+、CD4+/CD8+A组较治疗前明显升高(P<0.01);B组较治疗前差异无统计学意义(P>0.05);A组治疗后血清CD3+、CD4+、CD4+/CD8+较B组治疗后变化的差值差异有统计学意义(P<0.01),而血清CD8+2组治疗前、后差异均无统计学意义(P>0.05).从治疗开始随访6个月,A组临床疗效明显优于B组(P<0.01).结论:大叶性MPP患儿存在细胞免疫功能紊乱,匹多莫德可调节患儿免疫平衡,增强患儿的免疫力,提高临床.

Effect of pidomode in the treatment of lobar Mycoplasma pneumoniae pneumonia in children

Abstract:Objective:To observe the clinical effects of pidomode in the treatment of lobar Mycoplasma pneumoniae pneumonia( MPP) in children,and evaluate its effect on cellular immune function. Methods:Forty-four children with lobar MPP were randomly divided into group A and group B,and 30 healthy children were set as control group. Group B were treated with azithromycin(10 mg·kg-1 · d-1 each time) for 3 courses of treatmemt,and the group A were treated with azithromycin combined with pidotimod(0. 4 g/d) for 2 months. The levels of T lymphocyte subsets in three group before treatment and after 2 months of treatment were detected using flow cytometry,and the clinical effect between group A and B was compared. Results:Before treatment,the serum levels of CD3 +,CD4 + and CD4 + /CD8 + in lobar MPP group significantly decreased,and the level of CD8 + increased compared with the control group(P<0. 01). After treatment,the serum levels of CD3 +,CD4 + and CD4 + /CD8 + in group A significantly increased compared with before treatment (P<0. 01),and the difference of which in group B between before and after treatment was not statistically significant(P>0. 05). After treatment, the differences of the serum levels of CD3 +, CD4 + and CD4 + /CD8 + between group A and group B were statistically significant(P <0. 01),and the difference of the level of serum CD8 + in two groups between before and after treatment was not statistically significant(P>0. 05). From the beginning of treatment to follow for 6 months,the clinical curative effect in group A was better than that in group B(P<0. 01). Conclusions:Cellular immune function disorder exists in lobar MPP,pidotimod can regulate the immune balance,strengthen immunity,and improve the clinical curative effect in children with lobar MPP.
