医院社区联动护理管理模式对脑卒中病人康复效果及生活质量的影响

目的:探讨医院社区联动护理管理模式对脑卒中病人康复效果及生活质量的影响。方法:随机将120例脑卒中病人分为观察组（n=60）及对照组（n=60），对照组应用常规性健康教育方式，观察组在对照组基础上应用医院社区联动护理管理模式实施健康教育，干预时间为6个月，比较2组病人干预前、干预后3个月、6个月肢体功能运动状态、日常生活活动能力、神经功能缺损情况及干预前及干预6个月后生活质量的变化。结果:干预后3个月和6个月，观察组病人肢体功能运动状态、日常生活活动能力评分均显著高于对照组（P<0.01），而神经功能缺损评分显著低于对照组（P<0.01）；2组干预后病人肢体功能运动状态、日常生活活动能力评分均高于干预前水平，神经功能缺损评分低于干预前水平（P<0.05~P<0.01）。干预后6个月，观察组物质生活、躯体功能、心理功能、社会功能及生存质量总评分均显著高于对照组（P<0.01）。结论:医院社区联动护理管理模式能有效促进脑卒中病人肢体功能恢复，减轻病人神经功能缺损，提高病人病人生活质量。

Effect of the hospital-community linkage nursing management mode on the rehabilitation and quality of life in patients with stroke

Objective:To explore the effects of the hospital-community linkage nursing management mode on the rehabilitation and quality of life in patients with stroke.Methods:One hundred and twenty stroke patients were randomly divided into the observation group and control group(60 cases each group).The control group was taught with the routine health mode,and the observation group were taught with hospital-community linkage nursing management mode based on the control group for 6 months.The functional status, activities of daily living,neurological deficit before intervention,after 3 and 6 months of intervention,and the quality of life before intervention and after 6 months of intervention between two groups were compared.Results:After 3 and 6 months of intervention,the scores of functional status and activities of daily living in observation group were significantly higher than those in control group(P<0.01),while the score of NIHSS in observation group was significantly lower than that in control group(P <0.01).The scores of functional status and activities of daily living in two groups after intervention were significantly higher than those before intervention,and the score of NIHSS in two groups after intervention was significantly lower than that before intervention(P<0.05 to P<0.01).After 6 months of intervention,the total scores of physical life,physical function,mental function,social function and social quality in observation group were significantly higher than those in control group(P <0. 01). Conclusions:The hospital-community linkage nursing management mode can effective promote the recovery of limb function,reduce neurological deficits,and improve the quality of life of patients with stroke.
