78例儿童肺炎支原体感染回顾性分析

目的:探讨儿童肺炎支原体(MP)感染的临床特点及诊治方法。方法:回顾性分析78例确诊为MP感染儿童的临床资料。结果:本组患儿以婴幼儿为主,3岁以内36例(46·2%),3~5岁27例(34·6%),5岁以上15例(19·2%)。婴幼儿以发热、肺部表现为主,其中表现为喘息者17例(21·8%)。MP感染并发肺外表现者以年长儿多见,且主要损害神经、血液、泌尿、心血管、消化系统,表现为脑炎、血小板减少性紫癜、急性肾炎、胸腔积液、心力衰竭及肝功能损害。结论:婴幼儿MP感染的发生率较高;不同年龄组的临床表现各异,有肺外表现者可涉及各个系统。提示以喘息和肺外表现为主要临床表现的患儿,应警惕MP感染,以免延误治疗。

Clinical retrospective analysis of 78 cases of children infected with mycoplasmal pneumonia

Objective:To explore the clinical characte ri stics, diagnosis and treatment of mycoplasmal pneumonia (MP) infection in childr en. Methods:The clinical data of 78 cases of children infected with MP were retrospectively analyzed. Results:Infants were the most of children in this group. Among them, 36 case (46.2%) aged within 3 years, 27 cases (34.6%) aged 3~5, 15 cases (19.2%) aged over 5 year. The main clinical manifestations of infants w ere fever and symptom of lungs, including 17 cases of wheezing (21.8%). MP inf ection complicated by manifestation out of lungs were more in older children, ma inly harmed nervous system, blood system, urinary system, vascular system, diges tion system, and showed encephalitis, thrombocytopenic purpura, acute glomerulon ephritis, pleural effusion, heart failure and liver function injury. Conclusion:The incidence of MP infection of infants is hig her. The clinical manifestation in different age groups is distinct, manifestat ion out of lungs can concern the various systems, which suggest that children wi th the main clinical manifestation of wheezing and manifestation out of lungs sh ould be on guard against MP infection to avoid delaying treatment.
