原发性肝癌CT“密度更低区”螺旋CT双期扫描表现

摘    要:目的 探讨原发性肝癌CT“密度更低区”螺旋CT肝双期扫描表现特点。方法 回顾性分析72例经临床、DSA及病理证实原发性肝癌CT“密度更低区”螺旋CT双期扫描表现。结果 根据其螺旋CT肝双期扫描表现,原发性肝癌CT“密度更低区”分为六个类型:(1)多发斑点状,30例;(2)片状并多发斑点状,14例;(3)片状并多发条状,11例;(4)多发条状并多发斑点状,8例。(5)单发类圆形3例;(6)混合型,6例。原发性肝癌CT“密度更低区”平扫及肝动脉期边界多欠清,门静脉期边界清楚,密度相对肝癌组织更低。肝癌CT“密度更低区”与其肝动脉期肿瘤内强化血管形态类型有一定关系。结论 螺旋CT肝双期扫描可较好显示原发性肝癌CT“密度更低区”形态、数量、部位,对原发性肝癌的鉴别诊断及介入治疗效果预测有一定临床意义。

Abstract:Purpose To discuss the CT feature of the "more hypodense areas" of primary liver cancer on the hepatic dual - phase CT scanning . Methods the hepatic dual-phase CT scanning date were reviewed retrospectively in 72 patients with primary liver cancer. Results According to the CT appearance, the CT "more hypodense areas" of primary liver cancer were divided into six types: (l)the mutiple flecks pattern in 30 cases (41.67% ). (2)the patches with mutiple flecks pattern in 14 cases(19.44%) . (3)the patches with multiple stripes pattern in 11 cases (15.28%). (4)multiple stripes with flecks pattern in 8 cases (11.11%). (5)single oval pattern in 3 cases(4.17%) . (6)mixed pattern in 6 cases(8.33%) .The CT "more hypodense areas"of primary liver cancer were poorly - defined margin on CT plain scan(n = 72) and hepatic artery - phase(n = 69). CT " more hypodense areas ' were well - defined margin and much too " more hypodense areas" on hepatic portal - phase. The CT " more hypodense areas of primary liver cancer were related to intratumor enhanced vessels shape on hepatic artery - phase. Conclusion Hepatic dual - phase CT scanning was an effective method in demonstrating the shape, number and position of CT "more hypodense areas" of primary liver cancer. It should have an improtant clinic value in evaluating the treatment effect and prognosis of hepatic arterial lipiodeol embolization of primary liver cancer and diagnosis for primary liver cancer.
