中西医结合治疗血管性痴呆35例临床观察

目的观察中西医结合治疗血管性痴呆(VD)的临床疗效。方法将70例血管性痴呆患者随机分为2组。治疗组35例采用中药辨证施治配合静脉滴注胞磷胆碱钠治疗,对照组35例采用口服都可喜配合静脉滴注胞磷胆碱钠治疗。于治疗前、治疗30日、治疗60日观察患者临床症状、神经系统体征、中医证候变化等,治疗前后以简易智能量表(MMSE)、Blessed行为量表(BBS)测定其神经、心理及智力变化情况。结果治疗组治疗后MMSE、BBS积分均有明显改善,与治疗前比较差异有统计学意义(P<0.05);与对照组治疗后比较,差异有统计学意义(P<0.05),治疗组明显优于对照组。治疗组临床疗效总有效率84.0%,对照组总有效率51.4%,2组比较差异有统计学意义(P<0.05)。治疗组中医证候疗效总有效率85.7%,对照组总有效率57.1%,2组比较差异有统计学意义(P<0.05)。结论中西医结合治疗对血管性痴呆疗效明显优于单纯西药治疗,对患者智能障碍的改善、社会生活能力、日常生活自理能力及个性改变均有良好作用。

Observation of vascular dementia treated by combination of TCM with Western medicine

Objective To observe the effect of combination of TCM with Western medicine on vascular dementia.Methods 70 patients with vascular dementia were randomly divided into two groups.35 patients in treatment group were treated by the treatment of TCM based on differentiation of symptoms and signs combined with intravenous drip of citicoline.35 patients in control group were treated by oral administration of duxil combined with intravenous drip of citicoline.Clinical symptom,nervous system sign,changes of syndrome of TCM before and after 30 days and 60 days of treatment.Changes of nerves,psychology and intelligence were measured by MMSE and BBS.Results Integration of MMSE,BBS were improved after treatment compared with those before treatment in treatment group(P<0.05).Improvement of MMSE,BBS in treatment group was superior to that in control group(P<0.05).Conclusion Effect of combination of TCM with Western medicine on vascular dementia was superior to simple Western Medicine.Combination of TCM with Western medicine has role to improve disturbance of intelligence,social life ability,self-care ability and personality.
