三维与二维高频超声技术应用于针灸穴位定位的可行性研究

摘    要:目的:应用三维及二维高频超声技术对猪肉进行针刺定位研究,以探讨三维及二维高频超声技术对新西兰大耳兔及人体针刺穴位定位的可行性。方法:①将针灸毫针刺入猪肉内,启用三维扫查,观察纵切、横切、冠状切面中毫针位置的同步变化,评价三维超声定位的准确性。②在猪肉内固定毫针A,针尖设定为靶心。二维高频超声引导毫针B至靶心30次,针灸医师盲穿毫针B至靶心30次,测量A与B两针尖之间的距离,统计分析医师盲穿的准确性。③毫针在二维高频超声探头引导下分别刺入4只新西兰大耳兔双侧足三里穴位,实时观察定位的精准性。④对15个人体不同穴位和30个同名穴位行超声探测,在二维高频超声引导下动态观察毫针针尖移动至穴位断面所定位置。结果:三维高频超声技术能准确反映刺入猪肉中毫针的完整空间构象并能精准定位。二维高频超声技术对毫针刺入猪肉指定位置、新西兰大耳兔足三里穴位、15个人体不同穴位及30个同名穴位均准确定位。结论:高频超声引导毫针对活体经穴断面解剖定位,具有精准、直观、动态、价廉的特点,可作为活体穴位定位的标准。

Study of locating acupuncture point by high frequency 3-dimensional and 2-dimensional ultrasonic technology

Abstract:Objective: High frequency 3-dimensional and 2 dimensional ultrasonic technology were applied on pork acupuncture research, discuss the location feasibility of New Zealand big ear rabbits and human body by this methods. Methods: ①The acu- puncture needle was pierced into the pork. The three dimensional probe was used to observe the needle position synchronously from longitudinal section, cross-section and coronal section. Evaluate the accuracy of three dimensional ultrasonic localization. ~The needle A was fixed into pork and its needlepoint was set as the bull's-eye. The needle B was guided by Ultrasound to the bull's-eye and the needle B was blind acupunctured to the bull's eye 30 times. The distance of two tips was measured. Analyze the accuracy of blind acupuncture. ②Acupuncture needles were pierced into Zusanli which located in the dorsolateral side of the double legs of four New Zealand big ear rabbits. The accuracy of the location of acupuncture needles were real-time, precisely observed. ③The 15 different acupuncture points and 30 same acupuncture points of 15 volunteers were detected by ultrasonog- raphy when the ultrasound sectional images in accordance with meridian anatomy sectional picture and the movement of acu- puncture needle pinpoint was observed by high-frequency ultrasound dynamically. Results: The piercing point of pork was pre- cisely located by 3-dimension high frequency ultrasonic technology. The needles spatial location can be displayed accurately. The piercing point of pork and the Zusanli of New Zealand big ear rabbits, 15 different acupuncture points and 30 same acu puncture points of 15 volunteers were accurately located by 2 dimensional high frequency ultrasonic technology. Conclusion: Locating the acupuncture point by high frequency ultrasonic technology has precise, intuitive, dynamic, simple, inexpensive characteristics and high clinical value. It should be used as the acupuncture point standard of human body.
