北京市15个社区2型糖尿病患者合并高血压率及血压达标的调查

目的 了解北京市社区2型糖尿病患者高血压的患病率、血压控制达标率.方法 2008年8月至2009年7月采用多极抽样方法与患者自愿原则,纳入北京市4个城区中15个社区卫生服务中心的2849例2型糖尿病患者为研究对象,行体格检查及相关的实验室检查.结果 2型糖尿病患者高血压的患病率为70.8％ （2016/2849）.入组前已合并高血压者1966例,其中血压＜140/90 mm Hg（1mm Hg=0.133 kPa）者占54.8％ （1077/1966）,血压达标（＜130/80 mm Hg）率17.8％（350/1966）.血压达标与未达标者间TG、空腹血糖、腰围、糖化血红蛋白水平差异有统计学意义（F值分别为23.51、23.26、39.00和22.91,均P＜0.01）.1198例行尿微量白蛋白检测,糖尿病合并高血压者较单纯糖尿病者尿微量白蛋白升高更多（P＜0.01）.结论 北京社区2型糖尿病患者高血压的患病率较高,血压达标率低；应加强糖尿病合并高血压患者多重危险因素控制和有效的社区管理.

Prevalence of hypertension and blood pressure control rate in patients with type 2 diabetes mellitus in Beijing communities

Objective To survey the prevalence of hypertension and blood pressure control rate in patients with type 2 diabetes mellitus （T2DM） in Beijing communities.Methods Total 2849 patients with T2DM were selected by multi-stage sampling from 15 Beijing urban communities.The patients were surveyed with questionnaire,physical examination and laboratory tests were also performed.Results The prevalence of hypertension in T2DM patients was 70.8% （2016/2849）,and the patients with blood pressure 〈 140/90mm Hg （1 mm Hg =0.133 kPa） accounted for 54.8% （1077/1960）.The rate of blood pressure control （〈 130/80 mm Hg） was 17.8% （350/1966）; there were significantly difference in triglyceride,fasting glucose,waist circumference and glycosylated hemoglobin levels between patients with blood pressure controlled and those with blood pressure not controlled.Elevation of urinary albumin excretion rate was more common in T2DM patients with hypertension than that in T2DM patients without hypertension.Conclusions The prevalence of hypertension is high and the blood pressure control rate is low in T2DM patients of Beijingcommunities.Multi-risk factor control and effective community-based management for T2DM with hypertension should be emphasized.
