钙拮抗剂类药物对原发性高血压患者昼夜节律变化的影响

目的：研究钙拮抗剂对非杓型原发性高血压患者血压昼夜节律变化的影响。方法：191例原发性高血压患者随机分成5组，每组连续服用一种钙拮抗剂4周，服药前后24小时动态血压监测，观察每种药物对血压昼夜变异率的影响。结果：硝苯地平控释剂、非洛地平治疗后收缩压和舒张压由非杓型血压昼夜节律恢复到杓型；硝苯地平、尼群地平治疗后收缩压和舒张压昼夜节律均无明显改变；氨氯地平治疗后仅收缩压昼夜节律恢复到杓型。结论：针对患者血压昼夜节律不同，选择合理的药物治疗对患者的预后有十分重要的意义。

The effect of calcium antagonists on day and night rhythmic BP changes in patients with essential hypertension

Objective:To investigate the effect of calcium antagonists on day and night rhythmic BP changes in patients with non-dipper essential hypertension. Methods:One hundred and ninety-one cases of patients with essential hypertension were randomly divided into 5 groups. Patients of each group took a certain kind of calcium antagonist continuously for 4 weeks, and diagnesed with 24-hour ambulatory blood pressure monitor before and after taking the drug. The effect of ench kind of calcium antagonist on BPF was analyzed. Results:Treatment with nifedipine ( release control preparation ) or plendil could make circadian rhythms of SBP and DBP change to dipper kind, while treatment with nifedipine ( ordinery preparation ) or nitrendipine did not significantly change circadian rhythms of both SBP and DBP, treatment with amlodipin could only make circadian rhythms of SBP recover to dipper kind. Conclusion:It is very important to select proper treatment measure according to different circadian rhythms of blood pressure
