人工股骨头置换与锁定加压钢板治疗高龄不稳定型股骨粗隆间骨折的临床分析

目的：探讨在高龄不稳定型股骨粗隆间骨折治疗中人工股骨头置换与锁定加压钢板的临床疗效。方法选取78例高龄不稳定型股骨粗隆间骨折患者临床资料进行回顾性分析，随机将患者分为研究组（39例）和对照组（39例），研究组采用人工股骨头置换治疗，对照组采用锁定加压钢板治疗，对比观察两组患者手术时间、术中出血量、术后卧床时间、术后并发症与术后髋关节功能Harris评分。结果术后，两组手术时间、术中出血量对比，对照组优于研究组，差异有统计学意义（P＜0.05）；研究组术后卧床时间优于对照组，差异有统计学意义（P＜0.05）；两组术后并发症、术后髋关节功能Harris评分对比，差异无统计学意义（P＞0.05）。结论高龄不稳定型股骨粗隆间骨折治疗中，在严格掌握患者手术适应证情况下，可采用人工股骨头置换与锁定加压钢板治疗，均能减少术后并发症，促进患者早期功能恢复。

Clinical analysis of artificial femoral head replacement and locking compression plate in the treatment of unstable intertrochanteric fracture for elderly patients

Objective To explore the clinical efficacy of artificial femoral head replacement and locking compression plate in the treatment of unstable intertrochanteric fracture for elderly patients. Methods Clinical data of 78 patients with unstable intertrochanteric fracture was retrospectively analyzed and they were randomly allocated to a research group（39 patients） and a control group（39 patients）. The research group received the treatment of artificial femoral head replacement, and the control group received the treatment locking compression plate. Surgery time, the amount of bleeding during surgery, postoperative time in bed, postoperative complications and Harris score of postoperative hip joint functions were observed and compared between the two groups. Results Surgery time and the amount of bleeding during the surgery in the control group were better than those in the research group, and the differences were statistically significant（P 〈 0.05）; time in bed in the research group was better than that in the control group, and the difference was statistically significant（P 〈 0.05）; the differences of postoperative complications and Harris scores of postoperative hip joint functions between the two groups were not statistically significant （P〉0.05）. Conclusion In the treatment of unstable intertrochanteric fracture for elderly patients, under the condition of strictly mastering patients＆#39; operative indications, artificial femoral head replacement and locking compression plate can be applied, which are able to reduce postoperative complications and promote patients＆#39; early recovery of functions.
