两种方案治疗社区获得性肺炎的成本-效果分析

目的:比较哌拉西林钠他唑巴坦钠联用阿奇霉素(A组)和头孢他啶联用阿奇霉素(B组)两种方案治疗社区获得性肺炎(CAP)的费用与疗效。方法:采用回顾性调查方法,收集渝北区人民医院2014年1月至12月住院治疗CAP患者资料,筛选出A组和B组治疗方案患者资料。统计分析两种治疗方案的疗效、药品费用和不良反应,并计算成本-效果比(C/E)。结果:两种治疗方案临床有效率无显著差异(P>0.05);两组患者体温、咳嗽、肺部啰音、X线胸片、白细胞计数、中性粒细胞比例、细菌清除率等疗效指标无显著差异(P>0.05);两组患者不良反应发生率无显著差异(P>0.05);B组C/E值显著低于A组C/E值(P<0.05)。结论:哌拉西林钠他唑巴坦钠联用阿奇霉素与头孢他啶联用阿奇霉素两种方案用于我院CAP安全性和有效性相同,但后者具有更好的经济性。

Cost-efficacy of combination use of antibiotics in treatment of community acquired pneumonia

Objective:To compare the cost and efficacy between piperacillin sodium/tazobactam sodium combined with azithro-mycin (group A) and ceftazidime combined with azithromycin (group B) in treatment of community acquired pneumonia (CAP). Methods:Retrospective survey methodology was used to collect the clinical data of CAP inpatients who belonged to group A or group B in People’s Hospital of Yubei District of Chongqing City from January 2014 to December 2014. The therapeutic effect,adverse effect and drug cost were analyzed,and the cost-efficacy ( C/E) was calculated. Results:There was no difference in effective rate,body tem-perature,cough,lung rales,chest X-ray,white blood cell count,neutrophil rate and bacterial clearance rate between group A and group B (P>0. 05). C/E of group B was lower than that of group A (P<0. 05). Conclusion:Efficacy and safety of piperacillin sodium/tazobactam sodium combined with azithromycin is same to ceftazidime combined with azithromycin in treatment of CAP in our hospital, but the latter is more economic.
