原发性痛风患者存在肾脏排泄尿酸减少

目的 探讨中国原发性痛风患者是否存在肾脏排泄尿酸减少。方法 对56中国原发性痛风患者及43例正常对照人群的肾脏排泄尿酸情况进行了研究，观察指标包括肾功能、尿肌酐（Ucr）、尿尿酸（Uur）、24h尿尿酸总量（24h Uur）、肌酐清除率(Ccr)、尿酸清除率(Cur)、尿酸清除分数(FRUAC)、单位肾小球滤过率尿酸排出(EurGF)、肾小球尿酸负荷 (Flur)及尿尿酸与尿肌酐比值(Uur/Ucr)等。其中尿酸的测定采用尿酸酶法， 肌酐的测定采用三硝基苦味酸法。结果 原发性痛风患者的血尿酸、24h Uur及Flur均明显高于正常对照人群（p<0.05），而Cur及FRUAC则低于正常对照人群（p<0.05）。结论 以上研究结果提示中国原发性痛风患者存在肾脏排泄尿酸能力的降低。

Renal Underexcretion of Uric Acid Is Present in Patients With Primary Gout

Objective To explore if renal underexcretion of uric acid is present in Chinese patients with primary gout. Methods Serum samples and 24-hour urine samples obtained from 56 primary gout patients and 43 normal age-matched subjects were assayed for renal function, 24-hour uric acid urinary excretion(24h Uur), uric acid clearance(Cur), creatinine clearance(Ccr), fractional excretion of uric acid(FRUAC), excretion of uric acid per volume of glomerular filtration(EurGF), urinary uric acid to creatinine ratio(Uur/Ucr), and glomerular uric acid filtered load(Flur). Results Patients with primary gout showed hyperuricemia, high 24h Uur and Flur level, when showed lower uric acid clearance and fractional excretion of uric acid（p<0.05）. Conclusion Renal underexcretion of uric acid is present in Chinese patients with primary gout.
