摘    要:目的研究大面积深度烧伤早期切痂对全身炎症反应综合征(SIRS)的影响作用。方法选取我院2001年1月—2008年12月收治大面积深度烧伤患者48例,经早期切痂及综合救治,观察SIRS的发生率、多器官功能障碍综合征(MODS)的发生率、病死率。结果治愈43例(89.6%),死亡5例(10.4%),其中3例死于多脏器功能衰竭,1例死于呼吸衰竭,1例死于急性肾功能衰竭;符合SIRS诊断19例,发生率为39.5%;符合SIRS诊断的患者中,有3例发生了MODS,发生率为15.8%。结论大面积深度烧伤早期切痂可有效阻断SIRS的发生及发展,降低MODS的发生率及病死率,从而大大提高临床疗效,改善患者的生活质量。

Abstract:Objective To study the effect and the relationship of early emergency treatment for burn patients on systemic inflammatory response syndrome (SIRS).Methods The data of burn patients hospitalized in our hospital from Jan 2001 to Dec 2008 were collected,and the incidence of SIRS and mortality of multiple organ dysfunction syndrome (MODS) were retrospectively analyzed.Results The effective emergency treatments for burn patients such as helping them get through the shock stage smoothly,controlling infection timely,taking immunotherapy and treatment against mediators correlated in SIRS can significantly reduce the incidence of SIRS,and the mortality of MODS as well.Conclusion Prevention and cure of SIRS with early fully effective fluid resuscitation,adequate and effective application of antibiotics,specific immunotherapy and so on,can effectively block the occurrence and development of SIRS,reduce the incidence and mortality of MODS,thus greatly enhance clinical efficacy and improve the life quality of the patients.
