脊髓血管母细胞瘤的显微外科治疗

目的 探讨脊髓血管母细胞瘤的手术治疗策略.方法 回顾性分析40例血管母细胞瘤患者的临床特点、术前MRI表现、术中所见、手术切除情况、术后MRI表现及随访结果,并采用McCormick评分标准评价术前术后脊髓功能.结果 术后病理均证实为血管母细胞瘤.40例患者均行显微外科手术,镜下全切肿瘤.术后症状改善30例,无变化8例,加重2例,无手术并发症.结论 后正中入路是安全有效切除脊髓血管母细胞瘤的方法,术后大部分患者可以获得好的治疗效果,有症状者应积极手术.

Microsurgical treatment of intramedullary hemangioblastoma of spinal cord

Objective To investigate the therapeutic strategies of spinal cord hemangioblastoma.Method Clinical data of 40 patients with intramedually hemangioblastoma were analyzed.The function of spinal cord was evaluated before and after operation with McCormick scale.Results The diagnosis of hemangioblastoma was confirmed by postoperative pathology.All the 40 patients underwent complete excision under microscope.The postoperative neurological status was improved in 30 patients,remained unchanged in 8,and aggravated in 2.There was no other surgical complication.Conclusions Spinal cord hemangioblastoma can be safely and effectively removed via posterior approach.Postoperatively,clinical outcome is excellent in the majority of cases.Patients with symptoms should be operated actively.
