组织多普勒Tei指数评价肺动脉高压患者左心室整体功能的研究

目的探讨肺动脉高压（pulmonary arteria hypertension，PAH）对左心室功能的影响，为PAH患者的治疗提供依据。方法选取PAH患者20例为PAH组，健康志愿者20例为对照组。于心尖四腔切面利用脉冲波组织多普勒（tissue Doppler imaging，TDI）测量左室侧壁（LVLw）和室间隔（IVS）处二尖瓣环位点频谱，测量等容舒张期（IRT）、等容收缩期（ICT）、射血期（ET），并计算左心室Tei指数。结果与对照组（44．31＋3．58）mm]比较，PAH组左室舒张末径（LV）（36．17±5．46）mm]较小，差异有统计学意义（t=4．92，P〈0．01）；PAH组LVLW的ICT（72．72±19．20）ms]延长，Tei指数（0．69±0．17）增大，IRT（92．27±15．61）ms]延长，左室ET（245．81±47．05）ms]缩短，与对照组分别为（61．83±9．35）ms、0．45±0．25、（69．54±13．17）ms和（287．85±19．75）ms]比较，差异均有统计学意义（P〈0．01）；IVS处的ICT（72，63±12．35）ms]延长，Tei指数（0．71±0．17）增大，IRT（100．68±27．92）ms]延长，左室ET（247．62±33．05）ms]缩短，与对照组分别为（63．91±13．09）ms、0．48±0．08、（77．72±14．94）ms和（282．14±23．17）ms]比较，差异均有统计学意义（P〈0．05，P〈0．01）。结论PAH患者左心室整体功能受损。

Assessent of left ventricular function using Tei index in patients with pulmonary arterial hypertension

Objective To explore the impact of pulmonary arterial hypertension （PAH） on left ventricular function in patients with PAH and to provide the base of treatment for patients with PAH. Methods Forty subjects were divided into the case group （20 cases with PAH） and control group （20 healthy subjects）. Tissue Doppler imaging （TDI） was used to measure the values of isovolumic relaxation time （IRT）, isovalumic contraction period （ICT）, ejection period （ET） in interventricular septum （IVS） and left ventrieular lateral wall （LVLW） sites of mitral annular from the apical four-chamber view. Tel index was calculated. Results The value（36.17±5.46）mm] of LV in case group was significantly lower than that（44.31±3.58）mm] in control group （P〈 0.01）. The values of ICT （72.72±19.20）, （72.63±12.35） ms], Tel index （0.69±0.17, 0.71 ±0.17） and IRT （92.27±15.61 ）, （100.68±27.92）ms] in LVLW and IVS of case group were significandy higher than those of control groupLVLW： （61.83±9.35）ms, 0.45±0.25, （69.54±13.17）ms,IVS： （63.91 ±13.09）ms,0.48±0.08, （77.72±14.94）ms,respectively], but the value of ET （245.81 ±47.05）, （247.62±33.05）ms] was lower than that of control group （287.85±19.75） ,（282.14±23.17）ms （P〈0.05 and P〈0.01 ）. Conclusion Left ventricular global function is impaired in patients with PAH.
