地尔硫对高血压病人肱动脉血流动力学的影响

摘    要:目的 :研究地尔硫缓释片对高血压病人肱动脉血流动力学的影响。方法 :5 5例高血压病人随机进入地尔硫缓释片治疗组或美托洛尔治疗组 ,用脉冲多普勒法测定肱动脉功能参数。地尔硫组 32例 ,男性 2 0例 ,女性 12例 ,年龄 4 9a±s8a(4 2~ 60a) ,每日口服地尔硫 90~ 180mg ;美托洛尔组 2 3例 ,男性 13例 ,女性 10例 ,年龄 5 0a± 10a(4 0~ 60a) ,每日口服美托洛尔 5 0~ 10 0mg。结果 :治疗 10wk后 ,2组的血压下降 [SBP分别为 (17.7± 2 .5 )kPa ,(17.7± 2 .5 )kPa ;DBP为 (10 .8± 1.1)kPa ,(1.0 8± 1.1)kPa]幅度相同 ,地尔硫组的肱动脉扩张 [肱动脉管径为 (0 .4 2± 0 .10 )cm]、血流量增加为 (65 9± 2 0 1)mL·min- 1、前臂阻力和最小阻力减小 ,而美托洛尔组则无上述改变。结论 :地尔硫缓释片不仅可以降低血压 ,而且能改善肱动脉功能。

Abstract:AIM: To investigate the influence of diltiazem on the hemodynamics of branchial artery in patients with essential hypertension. METHODS:Fifty five patients with essential hypertension were randomly divided into diltiazem group and metoprolol group. In the diltiazem group, 32 patients (M 20, F 12; age 49 a± s 8 a)(42 60 a), were given diltiazem 90 180 mg·d -1 po . In the metoprolol group, 23 patients (M 13, F 10; age 50 a±10 a)(40 60 a), were treated with metoprolol 50 100 mg·d -1 po . The parameters of branchial arteries' hemodynamics were determined by pullsed doppler techniques before and after the treatment. RESULTS:After 10 wk treatment, all patients' blood pressure was decreased. But in diltiazem group the branchial arteries' internal dimeter(Dd), the mean blood flow velocity( V m), and the volumic blood flow( Q ) increased markedly, the forearm arteries' resistance( R ) and minimal resistance (R min ) decreased obviously, but in the metoprolol group, no obvious changes were found. CONCLUSION: Diltiazem may not only decrease the blood preseure but also improve the branchial arteries' hemodynamics.
