经椎间孔入路、椎板间入路脊柱内镜下髓核摘除术治疗老年L5/S1椎间盘突出症患者的临床效果

目的探究经椎间孔入路脊柱内镜下髓核摘除术(PETD)以及经椎板间入路脊柱内镜下髓核摘除术(PEID)治疗老年L5/S1椎间盘突出症患者的效果及对患者VAS、JOA评分的影响。方法选择本院收治的150例老年L5/S1椎间盘突出症患者作为研究对象,根据手术方法的不同将其分为PETD组和PEID组,每组75例。比较两组的治疗效果。结果PEID组的手术用时短于PETD组,术中出血量、透视次数少于PETD组(P<0.05);两组的下床活动时间、住院时间比较,差异无统计学意义(P>0.05)。治疗后,两组的VAS、ODI评分均低于治疗前,JOA评分高于治疗前(P<0.05);治疗后,两组的VAS、JOA、ODI评分比较,差异无统计学意义(P>0.05)。两组的治疗满意度比较,差异无统计学意义(P>0.05)。结论PETD与PEID治疗老年L5/S1椎间盘突出症患者均可获得较好的效果,其中PEID的应用能够更好地缩短患者的手术用时,减少术中出血量以及透视次数。

Clinical effect of percutaneous endoscopic transforaminal discectomy and percutaneous endoscopic interlaminar discectomy in the treatment of elderly patients with L5/S1 intervertebral disc herniation

Objective To explore the effect of percutaneous endoscopic transforaminal discectomy(PETD)and percutaneous endoscopic interlaminar discectomy(PEID)in the treatment of elderly patients with L5/S1 intervertebral disc herniation and its influences on VAS and JOA scores.Methods A total of 150 elderly patients with L5/S1 intervertebral disc herniation admitted in our hospital were selected as the research objects.According to the different surgical methods,the patients were divided into PETD group and PEID group,with 75 cases in each group.The therapeutic effects of the two groups were compared.Results The operation time in the PEID group was shorter than that in the PETD group,and the amount of intraoperative blood loss and fluoroscopy times were less than those in the PETD group(P<0.05);there were no significant differences in the time of getting out of bed and hospital stay between the two groups(P>0.05).After treatment,the VAS and ODI scores of the two groups were lower than those before treatment,and the JOA score was higher than that before treatment(P<0.05);after treatment,there were no significant differences in VAS,JOA,ODI scores between the two groups(P>0.05).There was no significant difference in the satisfaction of treatment between the two groups(P>0.05).Conclusion Both PETD and PEID can achieve good results in the treatment of elderly patients with L5/S1 intervertebral disc herniation.The application of PEID can better shorten the operation time,reduce the amount of intraoperative blood loss volume and the number of fluoroscopy.
