单侧多功能外固定架与加压钢板治疗肱骨干骨折49例疗效比较

目的:比较外固定架与加压钢板治疗肱骨干骨折的疗效以提高肱骨干骨折的治疗水平。方法:手术治疗49例肱骨干骨折,其中肱骨加压钢板内固定18例,外固定架固定31例骨折均为闭合性,比较两种手术方法的手术时间、住院时间、骨折愈合时间等疗效指标和桡神经损伤、骨不连、肩关节功能障碍等并发症发生情况。结果:与钢板内固定相比,外固定架固定操作手术时间短(P>0.01),骨折愈合时间提前(P<0.01),而住院时间两者之间没有统计学差异(P>0.05)。外固定架固定未发生骨不连,钢板有3例出现骨不连接。暂时的桡神经麻痹钢板与外固定架分别为2例、0例,肩功能障碍分别为3例、4例,所有病例未发生骨感染。结论:与钢板相比外固定架治疗肱骨干骨折的治愈率高,并发症少。

A Comparison Between The Locking Unilateral Axial Dynamic Fixator and The Plate for Treatment of Humeral Shafe Fractures

Objective:To compare the therapeutic efficiency between the locking unilateral axial dynamic fixator and the plate in treament of the humeral shaft fractures.Methods :49 patienets with humeral shaft fractures(close fractures)were divided into the plate group and the unilateral axial dynamic fixator group.18 patients were fixed with plate and 31 patients were fixed with unilateral axial dynamic fixator.They were compared in term of operation,hospitalization,fracture healing time and the occurrence of postoperative complications between the two groups separately.Results :By comparison with the plate group,the unilateral axial dynamic fixator group had a short operating time(P<0.01) and an earlier fracture union(P<0.01).However,there was no statistics differentce in the time of hospitalization between the two groups(P>0.05).Nonunion occurred on 3 cases in plate group.Radial nerve palsy occurred in 2 cases of plate group and 0 case of unilateral axial dynamic fixator group.The obstruction of shoulder joint function was happened on 3 cases of plate group and 4 cases of unilateral axial dynamic fixator group.No infection was happened in all cases.Conclusion: The clinical results of use of locking unilateral axial dynamic fixator are more excellent than plates in the treatment of humeral shaft fractures.
