异位妊娠两种手术治疗方式的成本构成评价

目的评估腹腔镜与开腹手术治疗异位妊娠的成本-效果。方法对同期住院的异位妊娠腹腔镜手术患者62例与开腹手术患者78例进行回顾性分析,比较两组手术效果、总成本及其构成。结果腹腔镜组手术时间短,术中出血少,输卵管保留率高,术后康复快,镇痛率低、肠功能恢复快、住院天数短、恢复工作早,各项指标与开腹组比较均有显著差异(P<0.01);腔镜手术组的住院费、手术费及非手术费用均高于开腹手术组(P<0.01),误工费用明显低于开腹手术组(P<0.01),总费用两组无统计学差异。结论腹腔镜手术治疗异位妊娠效果好,社会成本低,优于开腹手术。

Economic Evaluation of Operative Methods in Treatment of Ectopic Pregnancy

Objective To explore the cost-effectiveness of laparoscopy and laparotomy in the treatment of ectopic pregnancy. Methods 62 cases of ectopic pregnancy using laparoscopy in treatment and 78 cases of ectopic pregnancy using laparostomy in the same period were retrospectively analyzed. The operative effects, total cost and cost components were compared. Results Compared with the laparotomy group, the patients who underwent laparoscopy enjoyed a quicker operation,a higher oviduct reservation rate, a less blood loss and a better recovery. There are obviously differences between two groups （P0.01）. The hospitalization charge, operation cost, non-operation cost of laparoscopy group was higher than laparotomy group, but the off-work loss of laparoscopy group was less than laparotomy group significantly. The total costs of the two groups had no statistical difference. Conclusions The patients who underwent laparoscopy enjoy a better effect. According to the mean income level of employees in Huizhou,the social cost of laparoscopy in the treatment of ectopic pregnancy is less than laparotomy. Laparoscopy may be better than laparotomy in the treatment of ectopic pregnancy.
