前路病灶清除椎间植骨联合内固定治疗脊柱结核

目的评价前路病椎切除同期植骨内固定治疗脊柱结核的临床效果。方法采用手术治疗胸、腰椎脊柱结核患者35例,其中经前路病灶切除、自体髂骨椎体间植骨并前方或侧前方内固定18例;经前路病灶清除、椎体间植骨并后路内固定13例;单纯前路病灶清除术4例。结果无术中、术后并发症,切口均一期愈合。随访1.5～2.5年,平均2年后凸畸形矫正,矫正角度丢失不明显。ESR于术后1个月逐渐下降,3个月后达到正常范围。随访期内病灶未见复发。骨性融合时间平均5个月结论对合适的病例,该法可有效稳定脊柱、维持其畸形的矫正;达到牢固地融合,效果明显。

Anterior Radical Debridement and Interbody Fusion Combined with Internal Fixation for the Treatment of Spinal Tuberculosi

To evaluate the clinical effect of treating spinal tuberculosis by anterior corpectomy and reconstrtcting with internal fixation. Methods：Thirty-five cases of spinal tuberculosis were treated with one-stage anterior radical debridement. Results：All patients were visited regularly after operation for 1.5-2.5 years,about 2 years in average .their focus was eliminated completely,the cut became healed for the firt stage.kyphosis was rectified . Conclusion：For some selected patients,this technique appears to be effective in stabilizing the spinal column,maintaining deformity correction,achieving bony fusion and improving clinical outcome.
