穿孔性阑尾炎的腹腔镜手术技巧

目的探讨穿孔性阑尾炎行腹腔镜阑尾切除术的技巧。方法2000年10月-2005年12月对112例穿孔性阑尾炎患者实施腹腔镜阑尾切除术(LA),对其临床资料进行分析。结果手术全部成功,无中转开腹,穿孔性阑尾炎术后均经病理证实,平均手术时间71.2min,术后排气时间21.6h,21例术后使用止痛药,16例置管引流,1例出现戳孔感染,无残余脓肿发生,平均住院时间5.2天。结论腹腔镜阑尾切除术在治疗穿孔性阑尾炎中具有创伤小、恢复快、并发症少和住院时间短等优点,熟练的手术技巧是手术成功的保证。

Surgical technique experience of laparoscopic appendectomy in perforative appendicitis

Objective To explore the technique of laparoscopic appendectomy(LA) in perforating appendicitis. Methods 112 patients with perforating appendicitis were treated with laparoscopic appendectomy from October 2000 to December 2005 and the clinical data were analyzed retrospectively. Results All operations were succeed and no one was converted to open surgery. Perforating appendicitis were confirmed by biopsies. The mean length of operation time was 71.2 minuts. The break wind time was 21.6 hours.21 cases were treated with antalgesic after operation. The druinage tube was put in 16 cases. 1 case had trocar pore infection.no case of residual abscess was foumd. The mean hospitalization was 5.2 days. Conclusions Laparoscopic appendectomy possesses mamy advantages,including minimal invasion, early recovery, few complications,short hospitalization time and the skillful technique is successful assurance.
