全直肠系膜切除术治疗中低位直肠癌的临床疗效观察

目的分析比较全直肠系膜切除术和传统手术方法治疗中低位直肠癌的临床疗效。方法选取2006年1月至2011年1月间我院收治的并行全直肠系膜切除术治疗的中低位直肠癌患者30例为观察组,以及同期行传统手术切除治疗的中低位直肠癌患者30例为对照组,比较两组治疗的临床效果。结果两组间患者在吻合口瘘发生率和住院时间上比较,差异无统计学意义（P〉0.05）,观察组在手术时间、术中出血量、术后感染以及局部复发率方面得到显著改善,患者5年生存率显著提高,两组间比较,差异均具统计学意义（P〈0.05）。结论全直肠系膜切除术较之传统手术方法治疗中低位直肠癌具更好的临床效果,能显著降低术后的局部复发率,提高患者的生存率,改善患者生活质量。

Observation on the Clinical Effects of Total Mesorectum Excision in the Treatment of Middle and Lower Rectal Carcinoma

Objective To analyze and compare the clinical effects of total mesorectum excision and traditional surgical method in the treatment of middle and low rectal carcinoma.Methods 30 patients with middle and low rectal carcinoma treated by total mesorectum excision in our hospital from January 2006 to January 2011 were selected as observation group and 30 case treated by traditional surgical method were selected as control group.The clinical effects of the two groups were analyzed and compared.Results The differences of stoma fistula incidence and hospital staying days between the two groups were not significant（P 0.05）.The operation time,bleeding amount,the post-operation infection incidence and recurrence rate in the observation group were improved significantly（P 0.05）.The survival rate within 5 years of observation group increased significantly（P 0.05）.Conclusions Compared with traditional surgical method,total mesorectum excision has better clinical effect in the treatment of middle and low rectal carcinoma,which can lower the postoperative local recurrence rate,improve patients＇ survival rate and quality of life.
