社区高血压综合管理近期效果评价

目的:评价社区高血压综合管理模式实施1年后的近期效果。方法:345例高血压患者随机分为综合管理组172例和对照组173例。综合管理组接受高血压综合管理模式,对照组接受常规的高血压三级管理模式。比较两组患者在项目实施前和实施1年后在血压达标情况、良好生活习惯、健康行为、体质状况方面的变化。结果:综合管理组1年后在血压达标情况明显优于对照组(P<0.001),良好生活习惯、健康行为方面明显提高,并明显高于干预后的对照组(P<0.05);体质指数(BMI)和腰臀比显著下降(P<0.05)。结论:高血压综合管理模式较传统的高血压三级管理模式近期效果更显著,更适合于社区高血压的管理,积极开展和推广此种模式对社区慢性病管理具有较高的参考价值。

Evaluation of recently effect on community hypertension comprehensive management

Objective To evaluate the recently effect on comprehensive management mode implementing community hypertension for 1 year.Method For 345 cases with hypertension were randomly divided into the comprehensive management group （n = 172） and the control group （n = 173） .Patients in the comprehensive management group received hypertension comprehensive management mode for 1 year,patients in the control group received conventional three levels management mode for 1 year.Blood pressure standards,good life habit,health behaviors and physical condition changes were compared between two groups of patients in the project before and after 1 year.Results Those in the comprehensive management group reached blood pressure standard more easily than those in the control group （P〈0.001） ,and good life habit,health behavior improved significantly after intervention in the comprehensive management group than in the control group （P〈0.05） .Body mass index （BMI） and waist-hip ratio in the comprehensive management group decreased significantly than those in the control group （P〈0.05） .Conclusion Hypertension comprehensive management mode has a more significant recently effect than conventional three levels management mode,and is more suitable for the community management of hypertension.Community carrying and promoting the active mode is of high reference value of community management of chronic disease.
