眉周皮肤软组织缺损的个性化美学修复

摘    要:目的探讨眉周皮肤软组织缺损的个性化美学修复方法。方法对29例因各种原因所导致的眉周皮肤软组织缺损的患者进行美学修复,根据组织缺损的部位和大小,尽量避免供区及受区术后出现畸形及功能障碍,合理地个性化制定修复方法。眉上方随意皮瓣修复12例,皮下组织蒂皮瓣5例,双叶皮瓣3例,对偶皮瓣2例;皮瓣长1.2—3.5cm,蒂部宽1.0~2.2cm,长宽比例不超过2.5:1.0;耳后发际缘游离皮片移植修复7例,面积为1.0cm×1.5cm-2.5cm×3.5cm。结果本组所有患者切口均I期愈合,皮瓣及皮片完全成活,无血运障碍。随访3个月至5年,受区皮瓣部位与周围皮肤色泽协调一致,质地柔软,弹性良好。带毛囊的全厚皮片于术后3周左右出现毛发脱落,术后8—10周毛发再生,其走行方向与眉毛~致。结论眉周皮肤软组织缺损修复应根据其面积及部位个性化制定修复方法,从而获得较好的美学效果。

Individualized aesthetic repair of perieyebrow soft tissue defects

Abstract:Objective To explore the individualized aesthetic repair of perieyebrow soft tissue defects. Methods According to the defective area and size, perieyebrow soft tissue defects in 29 patients were underwent individualized aesthetic reconstructive operation to avoid postoperative malformation and dysfunction in both donor and recipient sites. Random flap above the eyebrow was applied in 12 patients, the subcutaneous pedicle skin flap was applied in 5 patients, the bilobed skin flap was applied in 3 patients, the double transpo- sition skin flap was applied in 2 patients, and. The length of the flaps ranged from 1.2 to 3.5 cm, whereas the pedicle width ranged from 1.0 to 2.2 cm, with the ration of length to width less than 2.5: 1.0. Full thickness skin graft with hair follicle was applied in 7 patients, which area ranged from 1.0 cm × 1.5 cm to 2.5 cm × 3.5 cm. Results All cases obtained satisfactory results functionally and aesthetically. All the flaps survived completely without any complications. The follow-up period was from 3 months to 5 years. The flaps provided excellent match in skin color and texture. All cases achieved good aesthetic results in both the donor and the re- cipient sites. The hair in full thickness skin graft with hair follicle shed at 3 weeks postoperatively, whereas regenerated at 8 to 10 weeks postoperatively, and the orientation were in accord with eyebrow. Conclusion Individualized therapeutic methods should be designed according to the position and size of perieyebrow soft tissue defects rationally to acquire ideal results.
