高血压性脑出血早期手术临床探讨：附44例报告

我科自１９９０年１月至１９９１年１２月，共收治高血压性脑出血患者４４例，其中男性２８例，女性１６例，年龄４０岁至６９岁，昏迷８例，中度昏迷２０例，中度昏迷伴有脑疝１３例，深昏迷伴有去脑强直３例。ＣＴ证实丘脑出血１３例，壳核１１例，基底节１４例，脑叶４例，小脑２例，其中血肿破入脑室１０例。血肿小量２０ｍｌ，大量１００ｍｌ，６０ｍｌ占本组５０％，发病至手术时间３小时内必例，２４小时２８例，４８小时１２例，采取中骨窗大骨瓣血肿清除止血减压，置引流管。本组结果，１２例痊愈胜任轻工作，１４例生活自理，８例需照料扶持，１０例因并发症再出血死亡。超早期，早期手术是挽救病人有效措施，优于药物治疗。

Clinical Survey of Hydertonic Cerebral Hemorrhage Early Operation

from 1990 jan. to 1991 dec. totally admitted hppertonic cerebral hemorrhge 44 cases. among them: men 28cases. women 16 cases; age 40- 69; coma 8 cases .middle coma 20 cases .middle coma with cerebral hernia 13 cases,deep coma with de-cerebral spasm 3 cases. et proved thalamic hemarrhace 13 cases, putamen 11 cases.nucleus basialis 1 4 cases.cerebral fol iage 4 cases .cerebellum 2 cases,hematoma Into cerebral room 10 cases; less hematoma 20ml. bigest hematoma 100ml,more than 60ml 50% in this group; time from hemorrhage to operaton: within 3 hours 4 cases, within 24 hours 28 cases; within 48 hours 1 2 cases. middle bone windowing large bone valve hematoma clearance and hemostasis. dramage tube -putting techique are performed. result: 1 2 cases healed and could dolicht wouk: 1 4 cases could lead a self-supporting life; 8 cases could be helped by others; 10 cases died of rehemorrhage and complications. immediate or early opoeration is an effective measture to save more hyportonic cerebral hemorrhage patient and is beder than medicine therapy.
