| 摘 要： | 目的探讨慢性化脓性中耳乳突炎外科手术的合理选择和术式改进。方法对20例慢性化脓性中耳乳突炎患者施行完整骨桥式乳突手术（IBM）合并后鼓室径路，开放上鼓室、鼓窦及乳突，磨薄削低骨性外耳道后壁，经面神经隐窝开放后鼓室，保留低位骨桥，去除不可逆病灶，同期或分期进行鼓室成形术。结果随访1年，干耳时间5～13周，平均7周，术前、术后30d平均听阈比较差异有显著性（t=9．06，P〈0．01）。术前与术后1年平均听阈比较差异有显著性（t=8．15，P〈0．01）。结论在根除病灶的基础上保存并提高听力，IBM合并后鼓室径路为慢性化脓性中耳乳突炎外科手术的合理选择。

| Abstract: | Objective To discuss a surgical option for the treatment of chronic suppurative otitis media. Methods 20 patients of chronic suppurative otitis media underwent intact bridge mastot ymplasty(IBM),combined with posterior tympanum. Results The follow-up had been 1 year. The dry ear was obtained with an average of 7 weeks. Comparing pre-operation PTA with post-operation 30 days PTA, there was a significant difference(t=9.06, P<0.01). Comparing pre-operation PTA with post-operation 1 year PTA, there was a significant difference(t=8.15, P<0.01). Conclusion IBM combined with posterior tympanum is a good choice for preserving or improving hearing based on eradication of the focus among patients with chronic suppurative otitis media. |
