腹腔镜胆总管探查一期吻合术后病人最佳的进食时间探讨

目的]探讨腹腔镜胆总管探查一期吻合术后的最佳进食时间.方法]将140例腹腔镜胆总管探查术病人分为对照组和观察组,对照组于病人肛门排气后进食,观察组于病人全身麻醉清醒后即给予低脂流质饮食,比较两组病人术后肠蠕动恢复时间、肛门排气时间、胃肠不适症状发生率及并发症发生率、术后3 d静脉补液量、住院时间等.结果]与对照组相比,观察组胃肠道功能恢复快、术后胃肠不适症状及并发症发生率低、静脉补液量少、住院时间短,经比较差异均有统计学意义（P＜0.05）.结论]腹腔镜胆总管探查一期吻合术后病人全身麻醉清醒后即给予小量多次低脂流质饮食,可加速胃肠功能的恢复,减少术后恶心、呕吐不适反应,减少术后并发症及静脉补液量,利于病人健康恢复.

Objective：To probe into the best eating time after laparoscopic common bile duct exploration an anastomosis. Methods：A total of 140 cases of patients undergoing laparoscopic common bile duct exploration were divided into control group and observation group, and patients in control group ate after the anus exhaust, patients in experimental group were given a low- fat and liquid diet when awake after general anesthesia, then to compare the postoperative recovery time of bowel movement, anal exhaust time, the incidence of gastrointestinal symptoms and the incidence of com- plications,intravenous fluid volume on 3rd day after operation and length of stay. Results：Compared with the control group, the recovery of gastroin- testinal function was faster, incidence of postoperative gastrointestinal dis- comfort symptoms and complications was lower, intravenous rehydration was less, hospital stay was shorter in observation group, the differences were statistically significant （P~0.05）. Conclusions ： It＇ s safe, feasible and effective that several small low - fat liquid diet for patients after laparo- scopic common bile duct exploration an anastomosis and awake after general anesthesia,and it can accelerate the recovery of gastrointestinal function and reduce postoperative nausea and vomiting discomfort, reduce postoperative complications and reduce the amount of intravenous fluids, so it will help restore the health of patients.
