摘    要:分析应用标准大骨瓣减压术治疗额颞叶脑挫裂伤的疗效。方法:回顾性分析38例对冲性额颞叶脑挫裂伤患者进行标准大骨瓣开颅术的临床资料。结果:根据GOS分级标准,38例手术患者中存活30例(78.9%),其中恢复良好18例(47.5%),中度残废6例(15.8%),重度残废3例(7.8%),植物生存3例(7.8%),死亡8例(21.1%)。结论:标准外伤骨瓣开颅治疗对冲性额颞部脑挫伤伴血肿具有视野好、暴露充分、减压彻底等优点,在降低致残率,改善患者的预后、预防并发症的发生等方面均有优势,具有较强的临床实用性。

Abstract:Analysis of curative effect of clinical application of standard large trauma craniotomy on treatment of frontotemporal brain contusion and laeeration.Methods:Contrecoup frontotemporal contusion and lacera- tion of brain undergoing standard large trauma craniotomy in 38 cases were retrospectively analyzed.Results:Accord- ing to GOS classification, 38 cases of operation patients survive for 30 (78.9%) patients, including 18 cases with good recovery (47.5%), moderate disability in 6 cases (15.8%), severe disability in 3 cases (7.8%), plant survival of 3 cases (7.8%), 8 cases of death (21.1%).Conclusion:The standard large trauma craniotomy treatment of contrecoup contusion and laceration of the brain with frontotemporal is advantageous in nice view, full exposure, thorough decompression, reducing the rate of disability, improving the prognosis, prevention of complications and so on, which is clinically practical.
