螺旋CT胆道造影显示阻塞性黄疸患者尾状叶胆管

目的　观察螺旋CT(HCT)胆道造影对胆道系统 ,特别是尾状叶胆管的显影效果。材料与方法　临床 2 3例阻塞性黄疸患者行经皮肝穿刺胆道造影引流术及常规直接法胆道造影检查 ,然后将 5 %泛影葡胺经引流管注入胆管内行螺旋CT扫描及 2D、3D重建。结果　HCT胆道造影 2D、3D图像显示胆管级别平均为 3.6 9± 0 .5 6、3.6 4± 0 .6 3,显示尾状叶胆管 73支 ,高于常规胆道造影 ,二者具有显著性差异。结论　HCT胆道造影胆管显示较好 ,对尾状叶胆管显示能力强。

Visualization of Biliary Duct of Caudate Lobe with Helical CT Cholangiography in Obstructive Jaundice

Objective To observe the visualization of biliary system, especialy the bile duct of caudate lobe, with helical CT Cholangiography.Materials and Methods This study included 23 patients who had obstructive jaundice and underwent percutaneous transhepatic cholangiography and drainage (PTCD). All patients were initially examined with conventional cholangiogrpahy and CT scanning. After 5% meglumine diatrizoate was infused into the bile duct through a draining tube, the helical CT scanning was performed within one breath holding. The raw data was fed into a workstation for 2 D and 3 D reconstruction.Results The mean visualization grade of the biliary ducts on 2 D and 3 D images was 3.69(0.56 and 3.64(0.63, respectively. On 3 D cholangiographs, 73 caudate lobe branches were visualized, the demonstrating rate of the caudal branches was significantly higher than that with conventional cholangiographs.Conclusion Helical CT cholangiography is very useful in demonstrating the biliary ducts of the caudate lobe.
